针灸三通法配合脊柱局部推拿对颈肩腰腿痛患者的影响

摘　　要：目的观察针灸三通法配合脊柱局部正骨推拿对颈肩腰腿痛患者的疗效。方法按随机数字表法将郑州市颈肩腰腿痛医院2020年7月—2021年12月收治的颈肩腰腿痛患者111例分为3组,对照1组(37例)予针灸三通法,对照2组(37例)予脊柱局部正骨推拿,联合组(37例)予针灸三通法配合脊柱局部正骨推拿。比较3组疗效、疼痛程度、疼痛因子、炎症因子、复发率。结果联合组总有效率高于对照1组、对照2组(P<0.05)。治疗后,3组β-内啡肽(β-EP)均较治疗前高(P<0.05),且联合组较对照1组、对照2组高(P<0.05);3组视觉模拟评分法(VAS)评分、5-羟色胺(5-HT)、前列腺素E_(2)(PGE_(2))、C反应蛋白(CRP)、单核细胞趋化蛋白-1(MCP-1)均较治疗前低(P<0.05),且联合组较对照1组、对照2组低(P<0.05)。随访24周,联合组复发率低于对照1组、对照2组(P<0.05)。结论针灸三通法配合脊柱局部正骨推拿能缓解颈肩腰腿痛患者疼痛,调节疼痛因子、炎症因子,降低复发率,提升疗效。Objective To observe the curative effect of acupuncture and moxibustion Santon method combined with spinal local bone-setting massage on patients with neck,shoulder,waist and leg pain.Methods According to the random number table method,111 patients with neck,shoulder,waist and leg pain were grouped,among them,the control group 1(n=37) received acupuncture Santon method,the control group 2(n=37) received spinal local bone-setting massage,and the combined group(n=37) received acupuncture Santon method combined with spinal local bone-setting massage.The efficacy,pain level,pain factors,inflammatory factors and recurrence rate were compared.Results The total effective rate of combined group was higher than that of control group 1 and control group 2(P<0.05).After treatment,β-endorphin(β-EP) in 3 groups was higher than before treatment(P<0.05),and the combined group was higher than control group 1 and control group 2(P<0.05).Visual analogue scale(VAS) score,5-hydroxytryptamine(5-HT),prostaglandin E2(PGE2),C-reactive protein(CRP) and monocyte chemotactic protein-1(MCP-1) were all lower in 3 groups than before treatment(P<0.05),and the combined group was lower than control group 1 and control group 2(P<0.05).After 24 weeks of follow-up,the recurrence rate of combined group was lower than that of control group 1 and control group 2(P<0.05).Conclusion Acupuncture and moxibustion Santon method combined with spinal local bone-setting massage can relieve pain in patients with neck,shoulder,waist and leg pain,regulate pain factors and inflammatory factors,reduce recurrence rate and improve curative effect.
