摘 要:目的利用液相-质谱联用(liquid chromatograph-mass spectrometer,LC-MS)代谢组学技术,探索肺脾气虚型慢性鼻-鼻窦炎不伴有鼻息肉(chronic rhinosinusitis without nasal polyps,CRSsNP)患者与空白组的尿液中内源性代谢产物的差异以及口服益气通窍饮后肺脾气虚型慢性鼻-鼻窦炎(chronic rhinosinusitis,CRS)特异性内源性代谢物的变化情况,从而揭示肺脾气虚型CRS的辨证物质基础和治疗靶点。方法选取2019年4月—2021年10月,黑龙江中医药大学附属第一医院肺脾气虚型CRS患者32例作为治疗组,鼻中隔偏曲者20例作为对照组,采用LC-MS技术检测两组患者尿液中内源性代谢产物,对潜在标记物进行筛选以及代谢途径进行分析。结果与鼻中隔偏曲组相比,肺脾气虚组患者的尿液中的显著性差异代谢物有13种,与代谢物相关的代谢通路主要有柠檬酸循环、咖啡因代谢通路、赖氨酸降解通路、苯丙氨酸、酪氨酸和色氨酸的生物合成通路、D-谷氨酰胺和D-谷氨酸代谢通路、酪氨酸和色氨酸代谢通路,口服益气通窍饮8周后,代谢产物均显示出现回调。结论基于LC-MS的代谢组学方法可以很好地从内源性代谢物的变化过程反映CRSsNP的发生情况。益气通窍饮对尿液中13种的代谢产物及其代谢通路的调节,可能是其治疗CRS的机制和途径。Objective Using liquid chromatography-mass spectrometry(LC-MS)metabolomics technology,this study aims to explore the differences in endogenous metabolites in the urine of patients with lung and spleen Qi deficiency type of chronic rhinosinusitis without nasal polyps(CRSsNP)and the blank group,as well as the changes in specific endogenous metabolites of chronic rhinosinusitis with lung and spleen Qi deficiency type after oral administration of Yiqi Tongqiao Decoction(益气通窍饮),thus revealing the syndrome differentiation material basis and therapeutic targets of chronic sinusitis with lung and spleen Qi deficiency type.Methods Volunteers were selected from The First Affiliated Hospital of Heilongjiang University of Chinese Medicine from April 2019 to October 2021.Thirty-two of volunteers with chronic rhinosinusitis of lung and spleen Qi deficiency syndrome was from E.N.T.department and twenty with nasal septum deviation was from Physical Examination Center.Endogenous metabolites were detected in the urine of both groups of patients by LC-MS technique and the metabolomic data was analyzed by multivariate statistics using EZinfo 3.0 software for screening the potential markers as well as metabolic pathway analysis.Results There were 13 significantly different metabolites in the urine of volunteers in the lung and spleen Qi deficiency syndrome group compared to the nasal septal deviation group.Metabolic pathways related to metabolites mainly included citrate cycle,caffeine metabolism,lysine degradation,phenylalanine,tyrosine and tryptophan biosynthesis,d-glutamine and d-glutamate metabolism,tyrosine metabolism and tryptophan metabolism.Conclusion The LC-MS-based metabolomics approach provides a good response to the occurrence of CRSsNP from the process of endogenous metabolite changes.The regulation of 13 metabolites and their metabolic pathways in urine by Yiqi Tongqiao Decoction may be the mechanism and way of its treatment of chronic rhinosinusitis.
