云南吴氏扶阳学术流派辨治痛风的特色和理论探析

摘　　要：云南吴佩衡扶阳学术流派历经五代传承,在痹证的辨治方面积累了丰富的经验。流派第二代传承人、国家级名中医吴生元教授认为痛风可归属于中医痹证范畴,其基本病机为“脾虚湿阻,痰瘀互结”,疾病急性发作时的病机要点为“湿热壅盛、痹阻经脉”,据此提出了健脾渗湿和清热通络的治法,并创立了健脾渗湿方和清热通络方分别用于痛风非急性期和急性期的治疗。流派第三、四代传承人彭江云教授和李兆福教授则在此基础上进一步构建了以“清热通络-健脾渗湿”序贯疗法为主治疗痛风的辨治体系,经临床验证效果显著。该文从理论层面对流派的辨治思路及理论依据进行阐述,以期为痛风及其他慢性疾病的防治提供参考。WU Peiheng strengthening Yang academic school in Yunnan has been inherited for five generations,and he has accumulated rich experience in syndrome differentiation and treatment.In the prevention and treatment of gout,Professor WU Shengyuan,the second generation inheritor of the school and a national-level famous traditional Chinese medicine doctor,thought gout can belong to the category of Bi syndrome in traditional Chinese medicine,and the basic pathogenesis of gout was“spleen deficiency and damp stagnation,intermingled phlegm and blood stasis”.In addition,he also pointed out that the main pathogenesis in the acute attack of the disease was“damp-heat stagnation and obstruction of meridians”.Base on the above,he put forward the treatment methods of invigorating spleen and eliminating dampness and clearing heat and dredging collaterals,and established Jianpi Shenshi Recipe(健脾渗湿方)for non-acute gout and Qingre Tongluo Recipe(清热通络方)for acute gout.Professor PENG Jiangyun and Professor LI Zhaofu,the third generation inheritor and the fourth generation inheritor of the genre,further constructed a syndrome differentiation and treatment system for gout mainly based on the sequential therapy of“clearing heat and dredging collaterals and invigorating spleen and eliminating dampness”.This system of syndrome differentiation and treatment has been proved to be effective clinically.In this paper,the thinking and theoretical basis of syndrome differentiation and treatment of schools are expounded from the theoretical level,in order to provide reference for the prevention and treatment of gout and other chronic diseases.
