2型糖尿病阴虚证与尿AQP2、AQP7表达的相关性研究

摘　　要：目的观察2型糖尿病阴虚证临床特征,明确患者尿中水通道蛋白-2(aquaporin-2,AQP2)、水通道蛋白-7(aquaporin-7,AQP7)表达特点及相关因素对其的影响,以利于进一步探索2型糖尿病阴虚证病证结合证候特点。方法制定2型糖尿病临床观察表、2型糖尿病阴虚证辨证标准量表以及中医证候诊断标准量表,将2021年3月—2021年12月北京中医药大学东直门医院肾病内分泌科住院部34例2型糖尿病患者分为阴虚证组(观察组)和阳虚证组(对照组)。采集两组患者血生化标本及清晨第一次全部尿液并采用酶联免疫吸附法(enzyme linked immunosorbent assay,ELISA)检测尿AQP2、AQP7的表达水平。对病例的性别、年龄、病程等多因素进行校正,分析两组尿AQP2、AQP7的差异。结果经校正后,2型糖尿病阴虚证患者尿AQP2水平明显低于阳虚证患者,MD(95%CI)为-64.23(-93.59,-34.87)(P<0.01),而尿AQP7水平低于阳虚证患者,MD(95%CI)为-3.98(-9.77,1.80)(P>0.05)。结论该研究所纳入2型糖尿病阴虚证患者尿AQP2指标表达低于阳虚证患者,提示AQP2可能是2型糖尿病阴虚证特异性生物标志物。Objective To observe the clinical characteristics of Yin deficiency syndrome in type 2 diabetes and clarify the expression characteristics of aquaporin-2(AQP2)and aquaporin-7(AQP7)in the urine of patients with type 2 diabetes and the influence of related factors on them,so as to further explore the characteristics of the combination of disease and syndrome of Yin deficiency syndrome in type 2 diabetes.Methods The clinical observation table of type 2 diabetes,the standard scale for synddrome differentiation of Yin deficiency syndrome of type 2 diabetes and the standard scale for diagnosis of TCM syndromes were formulated.From March 2021 to December 2021,34 patients with type 2 diabetes in the inpatient department of nephrology and endocrinology department of Dongzhimen Hospital of Beijing University of Chinese Medicine were divided into Yin deficiency syndrome group(observation group)and Yang deficiency syndrome group(control group).The blood biochemical samples of two groups and the first morning urine samples were collected and enzyme-linked immunosorbent assay(ELISA)was used to detect the expression levels of urinary AQP2 and AQP7.The multiple factors such as gender,age and course of disease in the cases were collected and the differences in urinary AQP2 and AQP7 were analyzed between the two groups.Results After correction,the level of urinary AQP2 in type 2 diabetes mellitus with Yin deficiency syndrome was significantly lower than that in patients with Yang deficiency syndrome,MD(95%CI)=-64.23(-93.59,-34.87)(P<0.01),and the level of urinary AQP7 was lower than that of Yang deficiency syndrome,MD(95%CI)=-3.98(-9.77,1.80)(P>0.05).Conclusion In this study,the expression of urinary AQP2 in patients with type 2 diabetes mellitus with Yin deficiency syndrome is lower than that in patients with Yang deficiency syndrome,suggesting that AQP2 may be a specific biomarker of type 2 diabetes with Yin deficiency syndrome.
