摘　　要：目的:探讨胎盘生长因子(PLGF)联合脐血流S/D对子痫前期的预测价值。方法:采用病例对照研究,回顾性选取2020年4月—2021年4月就诊于我院产科并最终诊断为子痫前期的孕产妇52例作为研究组,并选取同期正常分娩的产妇122例作为对照组,将孕期划分为三个阶段,孕早期(12~20周)、孕中期(20~34周)、孕晚期(34~42周),收集并分析两组孕妇早中晚期的胎盘生长因子(PLGF)、孕中晚期脐动脉收缩末期最大血流速度(S)与舒张末期最大血流速度(D)比值(S/D),评估PLGF、S/D值单独及联合预测子痫前期的效能。结果:研究组血清PLGF在妊娠各阶段平均值均明显低于对照组(P<0.05),研究组脐动脉S/D平均值高于对照组,孕中期两组间比较差异无统计学意义(P=0.21),孕晚期两组间比较差异有统计学意义(P=0.047),血清PLGF联合脐动脉S/D值在孕中期的特异度、灵敏度、AUC(ROC曲线下面积)分别为80.0%、63.6%、0.664,孕晚期的特异度、灵敏度、AUC分别为71.9%、81.8%、0.804。结论:研究组妊娠各个时期PLGF值均低于对照组,可以单独作为子痫前期患者的预测指标,S/D值单独预测价值与PLGF联合S/D值的预测价值在孕晚期优于孕中期,推测与纳入研究组患者的病情严重程度有关。Objective:Discusses the predictive value of placental growth factor(PLGF)combined with umbilical cord blood flow S/D in preeclampsia.Methods:Using a retrospective case-control study,52 pregnant women who were diagnosed as preeclampsia in the obstetrics department of our hospital from April 2020 to April 2021 were selected as the study group.122 pregnant women who delivered normally in the obstetrics department of the hospital at the same period were selected as the control group.The pregnancy was divided into three stages:early pregnancy(12~20 weeks),second trimester(20~34 weeks)and third trimester(34~42 weeks).The placental growth factor(PLGF)and the ratio of umbilical artery end-systolic blood flow velocity(S)to end-diastolic maximum blood flow velocity(D)in the early pregnancy and second trimester and third trimester were collected and analyzed,and the predictive value of PLGF and S/D alone or combined in predicting preeclampsia was evaluated.Results:The mean value of serum PLGF in the study group was significantly lower than that in the control group in each stage of pregnancy(P<0.05),and the mean value of umbilical artery S/D in the study group was higher than that in the control group,but there was no statistical significant difference between the two groups in second trimester(P=0.21),and there was statistical significant difference between the two groups in third trimester(P=0.047).The specificity,sensitivity and AUC(area under ROC curve)of serum PLGF combined with umbilical artery S/D were 80.0%,63.6%and 0.664 respectively in the second trimester.The specificity,sensitivity and AUC in the third trimester were 71.9%,81.8%and 0.804 respectively.Conclusion:The PLGF value of the study group during the whole pregnancy is lower than that of the control group.Single item can be used as a predictor of preeclampsia.The predictive value of S/D value alone and PLGF combined with S/D value in the third trimester is better than that in second trimester,which is related to the severity of the patients in the study gro
