摘　　要：目的探讨居家药学服务用于妊娠期和哺乳期药物治疗管理的效果。方法徐州市卫生健康委员会于2022年7月正式启动居家药学服务示范站点,以妊娠期和哺乳期妇女为服务对象,采用线下模式(家庭药师工作室、入户药学服务、药学门诊、讲座和义诊、编撰科普材料)与线上模式(居家药学服务微信群、网络直播、线上科普宣传)相结合的方式开展居家药学服务,统计服务范围,并评价服务结果。结果构建了徐州市妊娠期和哺乳期居家药学服务模式。2022年12月至2023年10月共签约妊娠期和哺乳期妇女及儿童(家属)1123例。全程药学监护下210例妊娠期妇女顺利完成分娩,未发生药品不良事件,妊娠结局均良好。规避和纠正签约对象居家治疗错误用药1600余次,合理用药认知及用药依从性评分均明显提升(P<0.05),签约对象对居家药学服务均非常满意。结论妊娠期和哺乳期居家药学服务可助力该阶段妇女及儿童的安全合理用药,提高区域优生优育水平,具有较强的可操作性和推广性。Objective To investigate the application effect of home-based pharmaceutical care in the medication therapy management during pregnancy and lactation.Methods The demonstration project of home-based pharmaceutical care for the women during pregnancy and lactation was started by the Xuzhou Municipal Health Commission in July 2022,the home-based pharmaceutical care was carried out by a combination of offline modes(home-based pharmacist office,in-home pharmaceutical care,pharmaceutical clinics,lectures,free clinics,compilation of popular-science materials)and online modes(WeChat groups of home-based pharmaceutical care,webcast,online science popularization and promotion).The service scope was recorded and the service effect was evaluated.Results The home-based pharmaceutical care mode for women during pregnancy and lactation in Xuzhou was constructed.A total of 1123 women during pregnancy and lactation and children(family members)signed the contract from December 2022 to October 2023.A total of 210 pregnant women who received whole-course pharmaceutical care successfully delivered,no adverse drug event occurred,and they all had good pregnancy outcomes.Over 1600 times of medication errors in women during pregnancy and lactation and children(family members)who signed the contract were avoided and corrected during home-based treatment,their awareness on rational drug use and medication adherence score significantly improved(P<0.05),and they were all very satisfied with home-based pharrmaceutical care.Conclusion Home-based pharmaceutical care for pregnancy and lactation can promote the safe and rational drug use of women at these stages and children,improve the regional healthy-birth and child-development level,which is operable and generalizable.
