李海霞治疗恶性肿瘤术后血液高凝状态经验

摘　　要：恶性肿瘤术后血液高凝状态严重威胁肿瘤患者的生命,干预恶性术后肿瘤血液高凝状态,能够有效降低患者死亡率。其临床表现与中医学中“血瘀证”的概念相接近。研究表明在术后抗凝、放置滤器、溶栓等治疗的基础上,配合中医药治疗疗效显著。李海霞教授对恶性肿瘤术后血液高凝状态的病因病机进行归纳:其一是癌症患者本身正气亏虚,久病伤阳,五脏六腑功能失调,此为根本;其二是肿瘤患者本身即有气血不足,手术后更伤气血,血脉不和,血不归经;其三为情志内伤,心神失司,肝气失于疏泄,气滞血瘀。治疗用附子、干姜、人参等温阳益气调和五脏;以神曲、麦芽、砂仁、白术等顾护健运脾胃;桃仁、红花、水蛭等活血理血,祛瘀生新;用桂枝、柴胡等条达情志,理气行滞。李教授基于此辨治原则,治疗恶性肿瘤术后血液高凝状态,在中药治疗的同时,嘱患者以小米粥恢复胃气,药食并行,取得了显著的临床疗效。The blood hypercoagulation state after malignant tumor operation seriously threatens the life of tumor patients.Intervention of blood hypercoagulation state after malignant tumor operation can effectively reduce the mortality of patients.Its clinical manifestations are close to the concept of“blood stasis syndrome”in traditional Chinese medicine.Studies show that on the basis of postoperative anticoagulation,filter placement,thrombolytic therapy and other treatments,combined with traditional Chinese medicine treatment have significant effect.Professor LI Haixia summarized the etiology and pathogenesis of postoperative hypercoagulability of blood after malignant tumor.First,the cancer patients have deficiency of vital Qi.Long-term illness causes Yang injury and the dysfunction of viscera and organs.The second is that the tumor patients are deficient in Qi and blood.After surgery,there is more injury to Qi and blood and disharmony between Qi and blood,and the blood does not return to the meridian.The third is emotional internal injury,loss of mind,liver Qi failing to soothe,Qi stagnation and blood stasis.The treatment uses Fuzi(Aconiti Lateralis Radix Praeparata),Ganjiang(Zingiberis Rhizoma)and Renshen(Ginseng Radix Et Rhizoma)to warm Yang,benefit Qi and regulate the five Zang-organs.Shenqu(Massa Medicata Fermentata),Maiya(Hordei Fructus Germinatus),Sharen(Amomi Fructus)and Baizhu(Atractylodis Macrocephalae Rhizoma)are used to strengthen spleen and stomach.Taoren(Persicae Semen),Honghua(Carthami Flos)and Shuizhi(Hirudo)can activate blood circulation and regulate blood.Guizhi(Cinnamomi Ramulus)and Chaihu(Bupleuri Radix)can regulate the emotion and regulate and relive Qi stagnation.Professor LI treated the hypercoagulable state of blood after malignant tumor.In addition to traditional Chinese medicine,he ordered the patients to restore stomach Qi with millet congee,which have achieved remarkable clinical effect.
