复方苦参注射液在三阶梯止痛的疗效分析

摘　　要：目的探讨复方苦参注射液用于WHO三阶梯止痛原则中的止痛价值。方法随机选取医院2019年7月—2022年4月150例癌痛患者,按照随机数字表法分为治疗组75例,对照组75例。对照组轻度癌痛患者使用布洛芬缓释片,中度癌痛患者使用盐酸曲马多缓释片联合布洛芬缓释片,重度癌痛患者使用盐酸吗啡缓释片联合布洛芬缓释片联合醋酸地塞米松片处理,治疗组在对照组基础上加用复方苦参注射液,21 d为1个疗程。观察比较两组治疗后的疼痛有效缓解率以及疼痛程度数字评估量表(numerical rating scale,NRS)、EORTC QLQ—C30(EuroPean organization for Research and Treatment)量表评分、匹兹堡睡眠质量指数与安全性。结果经治疗,治疗组有效缓解率升高(P<0.05);治疗组的NRS评分明显升高(P<0.01);EORTC QLQ—C30量表评分部分升高(P<0.05);匹兹堡睡眠质量指数明显升高(P<0.01);两组在治疗期间均出现不良反应,但治疗组不良事件发生率在恶心呕吐与便秘两方面更低(P<0.05);结果差异均有统计学意义(P<0.05)。结论在WHO三阶梯止痛原则基础上,加用复方苦参注射液可以明显提高止痛效果,明显改善生活质量,提高患者睡眠质量,同时也可降低不良事件发生情况,因此值得在临床推广应用。Objective To explore the analgesic value of compound Sophora Flavescus injection in WHO three-step analgesic principle.Methods 150 patients with cancer pain in our hospital from July 2019 to April 2022 were randomly selected as research objects,and were divided into treatment group(75 cases)and control group(75 cases)according to random number table method.Control group patients with mild pain use ibuprofen zyban,moderate pain patients with tramadol hydrochloride zyban joint ibuprofen zyban,severe pain patients with morphine hydrochloride zyban joint ibuprofen zyban dexamethasone acetate joint processing,on the basis of control group and treatment group with compound sophora injection,21 d for a period of treatment.The effective relief rate,Numerical Evaluation Scale of Pain Degree(NRS),EORTC QLQ-C30 scale score,Pittsburgh Sleep Quality Index and safety of the two groups were observed and compared.Results After treatment,the effective remission rate of the treatment group was increased(P<0.05).The NRS of the treatment group was significantly increased(P<0.01).The score of EORTC QLQ-C30 was partially increased(P<0.05);Pittsburgh sleep quality index was significantly increased(P<0.01);Adverse reactions occurred in both groups during treatment,but the incidence of adverse events in the treatment group was lower in nausea,vomiting and constipation(P<0.05).The results were statistically significant.Conclusion On the basis of WHO three-step analgesic principle,the addition of compound Flavescus injection can significantly improve the analgesic effect,significantly improve the quality of life,improve the sleep quality of patients,and reduce the occurrence of adverse events,so it is worthy of clinical application.
