摘 要:目的:观察负压封闭引流(VSD)联合威伐光(wIRA)治疗难愈性创面的临床效果。方法:选取2020年1月—2023年1月遂宁市中心医院收治的64例难愈性创面患者,按住院顺序排列,分为试验组和对照组,每组32例。试验组清创术后实施VSD+wIRA治疗,对照组清创术后实施换药治疗。观察两组肉芽生长速度、创面愈合速度、治疗时间以及临床换药次数。结果:试验组治疗第7天创面肉芽生长情况评分高于对照组,差异有统计学意义(P<0.05);试验组行Ⅱ期手术闭合创面的时间短于对照组,差异有统计学意义(P<0.05);试验组行Ⅱ期手术术前1d创面肉芽组织生长情况评分低于对照组,差异有统计学意义(P<0.05);试验组换药次数少于对照组,差异有统计学意义(P<0.05)。结论:VSD技术联合wIRA治疗难愈性创面的疗效确切,能缩短病程,是一种较理想的治疗方式。Objective To observe the clinical therapeutic effect of vacuum scaling drainage(VSD)combined with water-filtered infrared-A(wIRA)in the treatment of refractory wounds.Methods 64 patients with refractory wounds admitted to the Burn Suining Central Hospital from January 2020 to January 2023 were selected and arranged in hospitalization order,divided into an experimental group and a control group,with 32 cases in each group.After debridement,the experimental group received VSD+wIRA treatment,while the control group received conventional dressing changes.The growth rate of granulation tissue,wound healing rate,treatment time,and clinical dressing changes were observed in both groups.Results On the 7th day of treatment,the score of wound granulation growth in the experimental group was higher than that in the control group,and the difference was statistically significant(P<0.05).The time for the experimental group to undergo phase II surgery to close the wound was shorter than that of the control group,and the difference was statistically significant(P<0.05).The growth score of granulation tissue on the wound in the experimental group was lower than that in the control group 1 day before the second phase surgery,and the difference was statistically significant(P<0.05).The number of dressing changes in the experimental group was less than that in the control group,and the difference was statistically significant(P<0.05).Conclusions The combination of VSD technology and wIRA has a definite therapeutic effect on refractory wounds,can shorten the course of the disease,and is an ideal treatment method.
