摘　　要：目的:分析喉罩全身麻醉对年龄>70岁髋关节手术患者血流动力学的影响。方法:选取2019年12月—2021年12月信阳市人民医院骨科收治的104例年龄>70岁髋关节手术患者作为研究对象,按照麻醉方法分为两组,分别是实验组(52例,喉罩全身麻醉)和常规组(52例,气管内全麻)。比较两组患者术后6 h、术后12 h手术麻醉效果和采用视觉模拟评分(VAS),比较两组患者术中0.5 h、1 h血流动力学指标[平均动脉压(MAP)、心率(HR)]以及不良反应发生率。结果:实验组麻醉起效时间少于常规组,麻醉维持时间多于对照组,差异有统计学意义(t=13.809、5.005,P<0.05);术后6 h和术后12 h,实验组疼痛VAS评分低于常规组,差异有统计学意义(t=9.471、6.897,P<0.05);实验组患者MAP水平低于常规组术前、术中0.5 h,HR水平高于常规组,差异有统计学意义(t=7.854、6.694;t=8.241、3.522,P<0.05);实验组的术后发生不良反应率明显低于常规组,差异有统计学意义(χ^(2)=4.981,P<0.05)。结论:对于年龄>70岁髋关节手术患者,喉罩全身麻醉与气管内全麻相比,可改善髋关节手术麻醉效果,缓解疼痛,稳定血流动力学指标,降低不良反应发生率。Objective:To analyze the effect of laryngeal mask general anesthesia on hemodynamics in patients aged >70 years undergoing hip surgery.Methods:104 hip surgery patients aged >70 years admitted to the hospital from December 2019 to December 2021 were selected and divided into two groups according to the methods of anesthesia,which were the experimental group(52 cases,general anesthesia by laryngeal mask) and the conventional group(52 cases,general anesthesia by endotracheal tube).The effect of surgical anesthesia and the use of visual analog score(VAS) were compared between the two groups of patients6 h postoperatively and 12 h postoperatively,and the intraoperative hemodynamic indexes [mean arterial pressure(MAP),heart rate(HR)] as well as the incidence of adverse reactions were compared between the two groups 0.5 h and 1 h intraoperatively.Results:The onset of anesthesia in the experimental group was less than that in the conventional group,and the maintenance time of anesthesia was greater than that in the control group,with statistically significant differences(t=13.809,5.005;P<0.05).At 6h postoperatively and 12 h postoperatively,the pain VAS score of the experimental group was lower than that of the conventional group,and the difference was statistically significant(t=9.471,6.897;P<0.05).The MAP level of patients in the experimental group was lower than that of the conventional group in the preoperative and intraoperative 0.5 h,and the HR level was higher than that of the conventional group,and the difference was statistically significant(t=7.854,6.694;t=8.241,3.522;P<0.05).The rate of postoperative occurrence of adverse reactions in the experimental group was significantly lower than that in the conventional group,and the difference was statistically significant(χ^(2)=4.981,P<0.05).Conclusion:Laryngeal mask general anesthesia improves anesthesia for hip surgery,relieves pain,stabilizes hemodynamics,and reduces the incidence of adverse effects in hip surgery patients aged >70 years.
