特应性皮炎患者血清过敏原特异性IgE定量分析

摘  要:目的 定量分析特应性皮炎(AD)患者血清中常见的过敏原特异性IgE(sIgE),并与定性检测结果进行比较。方法 分析2019年12月至2021年12月间,国内15家医院进行血清过敏原sIgE检测至少一项阳性并愿意参加研究的154例AD组及209例非AD组湿疹皮炎患者(对照组)的血清sIgE水平。定量检测采用酶联免疫捕获法,包括10种食物(牛奶、鳕鱼、小麦、花生、大豆、蟹、虾、鸡蛋、牛肉、羊肉)及10种吸入(屋尘螨、粉尘螨、屋尘、柳树、猫上皮、狗上皮、蟑螂、普通豚草、艾蒿、交链孢菌)过敏原。通过计算血清总IgE升高率、血清特异性IgE的阳性构成比及水平浓度来比较两组之间的差异。结果 AD组血清总IgE水平为(229.69±37.18)IU/mL,显著高于非AD组[(167.75±22.56) IU/mL],差异有统计学意义(P<0.05)。而64.29%的AD组患者血清总IgE升高,与非AD组(60.77%)比较,差异无统计学意义(P>0.05)。AD组有6种过敏原牛奶、鳕鱼、小麦、大豆、蟹、鸡蛋的过敏原sIgE水平浓度为(25.35±3.62)、(20.55±1.70)、(4.05±0.37)、(5.46±0.92)、(14.88±2.80)、(12.96±2.55) IU/mL,均高于非AD组[(2.14±0.36)、(0.88±0.12)、(1.38±0.21)、(0.40±0.07)、(2.85±0.38)、(2.56±0.67) IU/mL],差异均有统计学意义(P<0.05)。而在阳性构成比中只有4种过敏原鸡蛋、牛奶、小麦、花生的(70.13%、38.96%、32.47%、5.84%)高于非AD组(39.23%、20.57%、16.27%、3.83%),差异均有统计学意义(P<0.05)。与定性检测结果比较,AD患者有更多的过敏原sIgE水平显著高于对照组。结论 过敏原定量检测较定性检测可以提供更多信息,更好为过敏管理提供参考。Objective To quantitatively analyze the common serum allergen-specific immunoglobulin E(sIgE) in patients with atopic dermatitis(AD),and to compare those with qualitative test results.Methods Serum sIgE levels of 154 AD and 209 non-AD eczema dermatitis patients(control group) who tested with at least one positive serum allergen sIgE from 15 hospitals in China between December 2019 and December 2021 and willing to participate in the study were analyzed.Quantitative detection was performed by enzyme-linked immunocapture method,which included 10 food(milk,cod,wheat,peanut,soybean,crab,shrimp,egg,beef,lamb) and 10 inhaled(house dust mite,dust mite,house dust,willow,cat epithelium,dog epithelium,cockroach,common ragweed,mug wormwood,Alternaria) allergens.The difference between the two groups was compared by calculating the increase rate of serum total IgE,the positive composition ratio of serum specific IgE and the level concentration.Results The serum total IgE level in AD group was(229.69±37.18) IU/mL,which was significantly higher than that in nonAD group [(167.75±22.56) IU/mL],and the difference was statistically significant(P<0.05).There was no significant difference between 64.29% of AD patients and 60.77% of control group(P>0.05).There were 6 kinds of allergens in the AD group:milk,cod,wheat,soybean,crab and egg.The sIgE levels were(25.35±3.62),(20.55±1.70),(4.05±0.37),(5.46±0.92),(14.88±2.80),(12.96±2.55) IU/mL,which were higher than those in the nonAD group [(2.14±0.36),(0.88±0.12),(1.38±0.21),(0.40±0.07),(2.85±0.38),(2.56±0.67) IU/mL],the difference was statistically significant(P<0.05).The positive proportion of four allergens in egg,milk,wheat and peanut(70.13%,38.96%,32.47%,5.84%) were higher than those in nonAD group(39.23%,20.57%,16.27%,3.83%),and the difference was statistically significant(P<0.05).Compared with the qualitative test results,more allergen sIgE level in AD patients higher than those of ontrol group.Conclusion Quantitative detection of allergens sIgE can provide more informa
