前列安通片联合体外冲击波治疗老年慢性前列腺炎的疗效观察  

Effect of Qianlie Antong Pian Combined with Extracorporeal Shock Wave Therapy on Elderly Patients with Chronic Prostatitis

摘　　要：目的探究前列安通片联合体外冲击波治疗老年慢性前列腺炎的疗效及对炎症因子、血液流变学的影响。方法按治疗方法不同,将116例老年慢性前列腺炎患者分为对照组(58例)和观察组(58例),分别予以体外冲击波治疗、前列安通片联合体外冲击波治疗。比较两组临床疗效及治疗前后慢性前列腺炎症状评分(CPSI)、尿流动力学[最大尿流率(Q_(max))、平均尿流率(Q_(av))]、炎症因子[白细胞介素-1β(IL-1β)、IL-8、肿瘤坏死因子-α(TNF-α)]、血液流变学[全血高切粘度(HBV)、全血低切粘度(LBV)、全血还原粘度(RV)]。结果观察组临床总有效率明显高于对照组(P<0.05)。治疗后,相比对照组,观察组CPSI评分显著下降(P<0.05),Q_(max)、Q_(av)g均明显提高(P均<0.05),前列腺液中IL-1β、IL-8、TNF-α水平及HBV、LBV、RV均明显降低(P均<0.05)。结论对于老年慢性前列腺炎患者,前列安通片联合体外冲击波治疗可缓解患者临床症状,改善尿流动力学,减轻局部炎症反应,促进血流循环,提高疗效。Objective To explore the efficacy of Qianlie Antong Tablets combined with extracorporeal shock wave in the treatment of chronic prostatitis in the elderly and its effect on inflammatory factors and blood rheology.Methods According to different treatment methods,116 elderly patients with chronic prostatitis were divided into a control group(58 cases)and an observation group(58 cases),which were treated with extracorporeal shock wave therapy and Qianlie Antong Tablets combined with extracorporeal shock wave therapy,respectively.The clinical efficacy,chronic prostatitis symptom index(CPSI),urodynamics[maximum flow rate(Q_(max)),average urinary flow rate(Q_(av))],inflammatory factors[interleukin-1β(IL-1β),IL-8,tumor necrosis factor-α(TNF-α)],hemorheology[whole blood high shear viscosity(HBV),whole blood low shear viscosity(LBV),whole blood reduction viscosity(RV)]before and after treatment were compared between the two groups.Results Compared with the control group,the total effective rate of the observation group was significantly higher(P<0.05).After treatment,compared with the control group,the scores of pain or discomfort symptoms,urination symptoms and quality of life in CPSI of the observation group were significantly lower(P<0.05).Q_(max)and Q_(av)g were significantly higher(P<0.05);the levels of IL-1β,IL-8 and TNF-αin the prostatic fluid were significantly lower(P<0.05),and HBV,LBV and RV were significantly lower(P<0.05).Conclusion Qianlie Antong Pian combined with extracorporeal shock wave can significantly relieve clinical symptoms,improve urodynamics,reduce local inflammatory reactions,promote blood circulation,and improve efficacy for elderly patients with chronic prostatitis.
