摘　　要：【目的】探讨腹腔镜下卵巢囊肿剔除术对患者卵巢功能、炎症因子及T淋巴细胞亚群水平的影响。【方法】选取2019年1月至2023年1月本院妇科收治的76例卵巢囊肿患者,根据手术方法不同分为对照组和观察组,每组38例。对照组行开腹卵巢囊肿剔除术治疗,观察组行腹腔镜下卵巢囊肿剔除术治疗,比较两组手术前后卵巢功能[雌二醇(E 2)、促卵泡激素(FSH)、黄体生成素(LH)]、炎症因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]及T淋巴细胞亚群水平(CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+))。【结果】两组术后3个月FSH、LH均升高,但观察组低于对照组(P<0.05);两组术后3个月E 2均降低,但观察组高于对照组(P<0.05)。观察组患者术后IL-6、TNF-α、CRP水平低于对照组,差异有统计学意义(P<0.05)。两组患者术前CD4^(+)、CD4^(+)/CD8^(+)比较,差异无统计学意义(P>0.05);术后1 d,两组患者血清CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均较术前明显降低,但观察组高于对照组,差异均有统计学意义(P<0.05);观察组患者术后3 d血清CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)恢复至术前水平。【结论】腹腔镜下卵巢囊肿剔除术可有效促进患者卵巢功能恢复,并减轻机体炎症反应,提高T淋巴细胞亚群水平,减轻对机体细胞免疫功能的抑制,有利于患者术后康复。【Objective】To explore the effects of laparoscopic ovarian cystectomy on ovarian function,inflammatory factors,and T lymphocyte subsets in patients.【Methods】A total of 76 patients with ovarian cysts admitted to the gynecology department of our hospital from January 2019 to January 2023 were selected and divided into a control group and an observation group based on different surgical methods,with 38 cases in each group.The control group underwent open ovarian cystectomy,while the observation group underwent laparoscopic ovarian cystectomy.Ovarian function[estradiol(E 2),follicle stimulating hormone(FSH),luteinizing hormone(LH)],inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)],and T lymphocyte subsets(CD3^(+),CD4^(+),and CD4^(+)/CD8^(+))before and after surgery were compared between the two groups.【Results】FSH and LH increased in both groups 3 months after surgery,but the observation group was lower than the control group(P<0.05);E 2 levels decreased in both groups 3 months after surgery,but the observation group was higher than the control group(P<0.05).The postoperative levels of IL-6,TNF-α,and CRP in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in preoperative CD4^(+)and CD4^(+)/CD8^(+)between the two groups of patients(P>0.05).On the 1st day after surgery,the serum CD3^(+),CD4^(+),CD4^(+)/CD8^(+)levels in both groups of patients were significantly lower than before surgery,but the observation group was higher than the control group,and the differences were statistically significant(P<0.05).The serum CD3^(+),CD4^(+),CD4^(+)/CD8^(+)levels in the observation group patients returned to preoperative levels 3 days after surgery.【Conclusion】Laparoscopic ovarian cyst removal surgery can effectively promote the recovery of ovarian function in patients,reduce the body's inflammatory response,increase the level of T lymphocyte subs
