摘  要:目的 探讨糖尿病对老年人接种新冠灭活疫苗后抗体水平的影响及完成两剂新冠灭活疫苗接种后30 d IgG抗体水平的影响因素。方法 选取2021年9—12月首都医科大学附属北京佑安医院接种新冠灭活疫苗的老年人55名,根据是否患糖尿病分为糖尿病组(13名)和非糖尿病组(42名)。采用胶体金法检测IgG、IgM抗体、假病毒中和实验检测中和抗体,比较两组完成两剂新冠灭活疫苗接种后30 d和180 d IgG抗体水平及完成加强针接种后7 d和14 d中和抗体几何均数滴度(geometric mean titer, GMT)水平,采用多重线性回归分析完成两剂新冠灭活疫苗接种后30 d IgG抗体水平的影响因素。结果 55例老年人中男21例,女34例,年龄60~76岁,平均(65.3±3.8)岁。完成两剂新冠灭活疫苗接种后30 d,糖尿病组IgG抗体水平及其抗体阳性率低于非糖尿病患者[1.28(0.21, 3.94)比4.47(2.48, 5.99)、53.85%比80.95%],差异均有统计学意义(P <0.05);两剂新冠灭活疫苗接种后180 d两组IgM、IgG抗体水平及其阳性率的比较,加强针接种后7 d和14 d两组抗野生株、抗BA.4/5变异株中和抗体GMT的比较,差异均无统计学意义(P> 0.05)。多重线性回归分析结果显示,糖尿病是IgG抗体水平的影响因素(P <0.05),IgG抗体随着患糖尿病而降低。结论 糖尿病可降低新冠灭活疫苗诱导的短期抗体水平,但不影响长期抗体水平。Objectives To explore the influence of diabetes on the antibody level of the elderly after inactivated vaccine against SARS-CoV-2 inoculation and the influencing factors of IgG antibody level 30 days after two doses of inactivated vaccine against SARS-CoV-2 inoculation.Methods A total of 55 elderly people in Beijing Youan Hospital from September to December 2021 were selected,and were divided into diabetes group(13 cases)and non-diabetes group(42 cases)according to the prevalence of diabetes.The IgG and IgM antibodies was detectd by colloidal gold method,and neutralizing antibody was detected by pseudovirus neutralization test.The IgG antibody levels at 30 d and 180 d after two doses of inactivated vaccine against SARS-CoV-2 inoculation and the geometric mean titer(GMT)levels at 7 d and 14 d after booster vaccination were compared between the two groups.The influencing factors of IgG antibody levels at 30 d after two doses of inactivated vaccine against SARS-CoV-2 inoculation were analyzed by multiple linear regression.Results Among 55 elderly people,there were 21 male and 34 females,aged from 60 to 76 years,with an average of(65.3±3.8)years.The IgG antibody level and antibody positive rate at 30 d and 180 d after two doses of inactivated vaccine against SARS-CoV-2 inoculation in diabetes group were lower than those in non-diabetes group[1.28(0.21,3.94)vs.4.47(2.48,5.99),53.85%vs.80.95%],and the differences were statistically significant(P<0.05).There was no significant difference in IgM and IgG antibody levels and their positive rates between the two groups at 180 d after inoculation of two doses of inactivated vaccine against SARS-CoV-2,and neutralizing antibody GMT between the two groups at 7 d and 14 d after booster inoculation(P>0.05).Multiple linear regression analysis showed that diabetes was the influencing factor of IgG antibody level(P<0.05),and IgG antibody decreased with diabetes.Conclusions Diabetes can reduce the short-term antibody level induced by inactivated vaccine against SARS-CoV-2,but it doe
