摘　　要：目的 探讨支撑喉镜下喉癌切除术后咽喉痛的严重程度及疼痛对生活造成的影响。方法 选取2021年9月至2022年8月首都医科大学附属北京同仁医院行支撑喉镜下喉癌切除术的患者100例,根据手术切除范围,单侧声带切除长度或双侧切除长度和≥1 cm为大范围组,<1 cm为小范围组,比较两组患者术中手术时间、麻醉总时间、丙泊酚用量、瑞芬太尼用量及不同时刻视觉模拟评分(visual analogue score, VAS),分析术后咽喉痛的程度及疼痛对患者生活造成的影响。结果 100例患者中,男91例,女9例;年龄41~68岁,平均(56.8±7.8)岁;手术时间为[48(41, 55)]min,麻醉时间为[67(61, 74)]min。大范围组手术时间、麻醉总时间、瑞芬太尼和丙泊酚用量、术后VAS,术后咽喉痛对情绪、睡眠和进食的影响均高于小范围组,差异均有统计学意义(P<0.05)。结论 支撑喉镜下喉癌大范围切除对术后12 h内的咽喉痛影响较大,且疼痛对术后患者的情绪、睡眠、进食造成轻度到中度的影响。Objective To investigate the severity of postoperative sore throat and the impact of pain on life after laryngeal cancer resection under suspension laryngoscope.Methods A total of 100 patients undergoing suspension laryngoscope laryngeal cancer resection under general anesthesia were selected from Beijing Tongren Hospital Capital Medical University from September 2021 to August 2022.According to the scope of surgical resection,the length of unilateral resection or bilateral resection≥1 cm were in the large group,and<1 cm were in the small group.The operation time,total anesthetic time,propofol dosage,remifentanil dosage and visual analogue score(VAS)at different time were compared between the two groups,and the degree of postoperative throat pain and the impact of pain on patients'life were analyzed.Results Among the 100 patients,91 were males and nine were females.They ranged in age from 41 to 68 years,with an average age of(56.8±7.8)years.The operation time was[48(41,55)]min,and the anesthesia time was[67(61,74)]min.Operation time,total time of anesthesia,dosage of remifentanil and propofol,postoperative VAS in large group were higher than those in small group,and the effects of postoperative sore throat on mood,sleep and eating were statistically significant(P<0.05).Conclusions Extensive resection of laryngeal carcinoma under suspension laryngoscopic surgery has a significant effect on laryngeal pain within 12 h after surgery,and the pain has a mild to moderate impact on the mood,sleep and eating of patients after surgery.
