摘 要:目的探讨糖尿病母亲所生新生儿血清可溶性CD163(serum soluble CD163,sCD163)的变化及临床意义。方法前瞻性择取2022年1月至2022年12月我院108例妊娠期糖尿病产妇及其所生新生儿纳入研究组,按新生儿出生体重分为巨大儿组(A组)和非巨大儿组(B组),同时纳入同期100例健康产妇及其所生新生儿作为对照组。检测各组脐血和母血可溶性CD163水平及相关血清指标。采用Pearson相关系数进行相关性分析及采用Logistic回归模型进行回归分析。结果研究组母血及脐血sCD163水平均显著高于对照组(P<0.05);A组脐血sCD163、INS、PG、TG、TCH、LDL-C、HDL-C、HCT及新生儿VAI水平均显著高于B组,同时A组母血sCD163和INS水平显著高于B组(P<0.05);研究组新生儿脐血sCD163表达与身长、出生体质量、出生重量指数PI、脐血TG、新生儿VAI和脐血HCT显著相关(P<0.05),而与胎龄、脐血INS和母血sCD163无显著相关性(P>0.05);多因素Logistic结果显示出生重量指数PI、脐血TG、HCT、VAI、脐血sCD163为巨大儿的独立危险因素。结论孕妇妊娠期罹患糖尿病可增加胎儿肥胖风险,而脐血可溶性CD163可反映胎儿生长发育情况,临床可针对高风险孕妇及时采取预防措施以降低此类孕妇心血管疾病和肥胖发生风险。Objective To investigate changes and clinical significance of serum soluble CD163(Serum soluble CD163,sCD163)in newborns among diabetic mothers.Methods From January,2022 to December,2022,108 pregnant women with diabetes mellitus and their newborn babies(the study group)were selected from our hospital.These subjects were divided into two groups according to the birth weight of the newborn:macrosomia group(Group A)and non-macrosomia group(Group B).During the same period,100 healthy pregnant women and their newborn babies were included in the control group.Levels of soluble CD163 in cord blood and maternal blood and related serum indexes were detected in each group.Pearson correlation coefficient was used for correlation analysis and logistic regression model was used for regression analysis.Results The maternal blood and cord blood sCD163 levels in the study group were significantly higher than those in the control group(P<0.05);Levels of cord blood sCD163,INS,PG,TG,TCH,LDL-C,HDL-C,HCT,and neonatal VAI in group A were significantly higher than those of group B,and the maternal blood sCD163 and INS levels of group A were significantly higher than group B(P<0.05);The expression of sCD163 in umbilical cord blood of the study group was related to body length,birth weight,birth weight index PI,cord blood TG,neonatal VAI and cord blood HCT were significantly correlated(P<0.05),while there was no significant correlation with gestational age,cord blood INS and maternal blood sCD163(P>0.05);Multivariate logistic results showed that birth weight index PI,cord blood TG,HCT,VAI,and cord blood sCD163 were independent risk factors.Conclusion Diabetes in pregnant women during pregnancy can increase the risk of fetal obesity,and serum soluble CD163 can reflect status of growth and development of the fetus.Clinical preventive measures can be applied in time for high-risk pregnant women to reduce the risk of cardiovascular disease and obesity.
