摘　　要：目的探讨无翅型小鼠乳房肿瘤病毒整合位点家族成员4(WNT4)基因在妊娠妇女中的表达变化及对早产的预测价值。方法采用便利抽样法,选取2019年1月至12月在本院分娩的80例妊娠孕妇为研究对象,根据分娩结局分为自发性早产组、足月组。比较两组孕妇一般资料及静脉血、脐带血WNT4表达水平变化,采用多因素Logistic回归分析进行自发性早产影响因素分析;绘制受试者工作特征曲线(ROC)评估WNT4表达水平对自发性早产的预测价值。结果自发性早产组与足月组分娩孕周、新生儿出生体重、C反应蛋白、宫颈内口应变平均值、宫颈长度、静脉血及脐带血WNT4表达水平比较,差异有统计学意义(P<0.05);多因素Logistic回归分析显示,宫颈长度、静脉血及脐带血WNT4表达水平为自发性早产危险因素(P<0.05);ROC曲线显示,静脉血及脐带血WNT4表达检测对自发性早产预测的灵敏度、特异性较高,且联合检查时灵敏度、特异性更高。结论相较于足月产孕妇WNT4基因在自发性早产孕妇中表达升高;临床检测孕妇静脉血及脐带血WNT4表达对预测早产有一定价值。Objective To investigate the expression of wingless mouse mammary tumor virus integration site family 4(WNT4)gene in pregnant women and its predictive value for preterm delivery.Methods Using the convenient sampling method,80 pregnant women who delivered in our hospital from January to December,2019 were selected as the research objects.These patients were divided into a spontaneous preterm delivery group and a full-term group according to the pregnancy outcome.The general data of pregnant women and the expression of WNT4 in venous blood and umbilical cord blood were compared between the two groups.The influencing factors of spontaneous preterm birth were identified by multivariate logistic regression analysis.Receiver operating characteristic curve(ROC)was drawn to evaluate the predictive value of WNT4 expression level in spontaneous preterm delivery.Results There were significant differences in gestational weeks of delivery,neonatal birth weight,C-reactive protein,internal ostium,cervical length,and expression of WNT4 in venous blood and umbilical cord blood between the spontaneous preterm birth group and full-term group(P<0.05).Multivariate logistic regression analysis showed that cervical length,the expression level of WNT4 in venous blood and umbilical cord blood were the risk factors of spontaneous preterm birth(P<0.05).ROC curve showed that the detection of WNT4 expression in venous blood and umbilical cord blood had a higher sensitivity and specificity for the prediction of spontaneous preterm birth,and the sensitivity and specificity of combined examination were even higher.Conclusion Compared with full-term pregnant women,the expression of WNT4 gene is increased in spontaneous preterm pregnant women.Detecting the expression of WNT4 in maternal venous blood and umbilical cord blood has a certain clinical value in predicting preterm delivery.
