摘  要:目的 探讨沙库巴曲缬沙坦纳片(诺欣妥)对慢性心力衰竭大鼠心功能及心肌组织AT1/CARP信号通路的影响。方法 100只Wistar大鼠分为正常对照组、模型对照组、依拉普利组、诺欣妥低剂量组、诺欣妥高剂量组,每组20只。采用心动仪测定各组大鼠心功能指标,全自动免疫分析仪测定各组大鼠血清NT-proBNP、Hs-CRP、血钾(K)水平,MK3酶标仪测定各组大鼠血清IL-4、IL-8、TNF-α、心肌组织P53、caspase-3蛋白水平,RT-PCR法及蛋白印迹法测定各组大鼠心肌组织AT1、CARP mRNA和蛋白水平。结果 与正常对照组比较,模型对照组、依拉普利组、诺欣妥各剂量组LVIDd、LVIDs、血清NT-proBNP、Hs-CRP、血K、IL-4、IL-8、TNF-α、心肌AT1、CARP mRNA和蛋白、心肌P53、caspase-3蛋白水平升高,FS、EF降低(P<0.05)。与模型对照组相比,依拉普利组、诺欣妥各剂量组大鼠LVIDd、LVIDs、血清NT-proBNP、Hs-CRP、血K、IL-4、IL-8、TNF-α、心肌AT1、CARP mRNA和蛋白、心肌P53、caspase-3蛋白水平降低,FS、EF升高(P<0.05)。结论 诺欣妥对慢性心力衰竭大鼠心功能具有明显改善作用,其机制可能与抑制AT1/CARP信号通路激活继而抑制心肌细胞凋亡有关。Objective To explore the effect of sacubitril valsartan sodium tablets(SVST)on cardiac function and AT1/CARP signaling pathway in myocardial tissue in rats with chronic heart failure.Methods One hundred Wistar rats were divided into normal control group,model control group,elapril group,SVST low dose group and SVST high dose group.The cardiac function indexes of rats were measured by echocardiography,serum NT-proBNP,Hs-CRP and serum potassium(K)levels were measured by automatic immunoassay analyzer,and serum IL-4,IL-8,TNF-α,P53 and caspase-3 protein levels in myocardial tissue were measured by MK3 enzyme standardization,and AT1,CARP mRNA and protein levels in myocardial tissue were measured by RT-PCR and western blot.Results Compared with normal control group,the LVIDd,LVIDs,serum NT-proBNP,Hs-CRP,blood K,IL-4,IL-8,TNF-α,myocardial AT1,CARP mRNA and protein,myocardial P53,and caspase-3 protein levels were increased in the model control,elapril group,and SVST dose groups,and FS and EF were decreased(P<0.05).Compared with the model control group,the LVIDd,LVIDs,serum NT-proBNP,Hs-CRP,blood K,IL-4,IL-8,TNF-α,myocardial AT1,CARP mRNA and protein,myocardial P53,and caspase-3 protein levels were decreased in the elapril group and SVST dose groups,and FS and EF were increased(P<0.05).Conclusion Noxantel can significantly improve cardiac function in rats with chronic heart failure,and its mechanism may be related to the inhibition of AT1/CARP signaling pathway activation and subsequent inhibition of cardiomyocyte apoptosis.
