摘　　要：目的:了解医养结合机构老年慢性病人群中医药服务获得感现状及其影响因素,为提升中医药服务获得感提供借鉴和参考。方法:于2023年7月,将HONGYY等编制的《社区老年人中医药服务获得感量表》转化为相应的问卷条目;于2023年8~9月,运用典型抽样法、简单随机抽样法在我国东、中、西部地区三家医养结合机构抽取690名60岁及以上患有慢性病老年人进行问卷调查;于2023年10月,采用统计描述、卡方检验、二元Logistic等统计学方法,对医养结合机构老年慢性病人群中医药服务获得感及影响因素进行分析。结果:共发放调查问卷690份,有效回收637份,有效回收率为92.32%(637/690)。医养结合机构老年慢性人群中医药服务获得感平均得分为3.99±0.58分,其中有效度、满意度和回应度及其8个条目得分均低于4分。获得感主要受户籍、患慢性病数量、接受中医药服务的情况、主要解决的健康问题等因素的影响,P<0.05。结论:医养结合机构老年慢性人群中医药服务获得感总体处于较高水平,今后应进一步加强有效度、满意度、回应度相关内容的工作力度,促进获得感五个维度的均衡发展。tObjective To understand the status and influencing factors of the sense of access to medical services among the elderly with chronic diseases in integrated medical and nursing care institutions,and provide reference and guidance for improving the sense of access to traditional Chinese medicine(TCM)services.Methods From August to September 2023,690 elderly people with chronic diseases aged 60 and above were selected from three integrated medical and nursing care institutions in the eastern,central,and western regions of China by using typical sampling and simple random sampling methods,and surveyed with questionnaire of"Perception of TCM Services for Community Elderly".Statistical methods such as statistical description,chi square test,and binary logistic were used to analyze the sense of medical service acquisition and its influencing factors among the elderly patients with chronic diseases in integrated medical and nursing care institutions.Results A total of 690 survey questionnaires were distributed,and 637 were effectively collected,with an effective response rate of 92.32%.The average score of TCM service acquisition for the elderly with chronic diseases in integrated medical and nursing care institutions was 3.99±0.58,in which all the scores of effectiveness,satisfaction,responsiveness,and their 8 item were lower than 4 points.The sense of gain was mainly affected by registered residence,number of chronic diseases,situation of receiving TCM services,health problems mainly solved and other factors.Conclusion The overall sense of access to TCM services for the elderly with chronic diseases in integrated medical and nursing care institutions is at a high level.In the future,efforts should be made to strengthen the related contents of effectiveness,satisfaction,and responsiveness,and promote the balanced development of the five dimensions of sense of access.
