摘　　要：目的了解脑卒中患者的夫妻疾病沟通体验。方法采用描述性现象学研究,使用目的抽样法,选取2022年8月—2023年3月在河南省某三级甲等综合医院神经内科住院治疗或复诊的19对脑卒中患者及其配偶进行半结构式访谈,采用Colaizzi 7步法分析资料。结果共提炼出2个主题和8个亚主题:建设性疾病沟通,包括开放沟通治疗信息、及时沟通症状变化、积极沟通二级预防和主动开导负性情绪;消极疾病沟通,包括回避沟通、沟通冲突、无效沟通和沟通隐瞒。结论脑卒中患者夫妻既存在建设性疾病沟通也存在消极疾病沟通体验。医护人员应开展针对性的干预,改善脑卒中患者夫妻疾病沟通方式,提升其共同决策能力,减少其负性情绪。Objective To gain insight into the experience of disease communication among couples with stroke.Methods A descriptive phenomenological approach was conducted and a purposive sampling method was used to select 19 couples with stroke from August 2022 to March 2023 in the Department of Neurology of a tertiary hospital in Henan Province for semi-structured interviews.Data were analyzed by the Colaizzi method.Results A total of 2 themes and 8 sub-themes were extracted.Constructive disease communication included open communication of treatment information,timely communication of changes in symptoms,active communication of secondary prevention,and proactive communication of negative emotions.Negative disease communication included avoidance of communication,communication conflict,ineffective communication,and communication concealment.Conclusion Couples with stroke experienced both constructive and negative disease communication.Healthcare professionals should develop targeted interventions to improve disease communication,enhance the ability of couples to make decisions together,and reduce their negative emotions.
