着痹方联合中药溻渍治疗痰饮流注型膝关节滑膜炎临床观察

摘　　要：目的观察着痹方联合中药溻渍治疗痰饮流注型膝关节滑膜炎的临床疗效。方法将92例痰饮流注型膝关节滑膜炎患者按照随机数字表法分为2组,对照组46例予依托考昔片治疗,治疗组46例在对照组治疗基础上予着痹方联合中药溻渍治疗,2组均治疗2周。比较2组疗效;比较2组治疗前后疼痛视觉模拟评分(VAS)、Lysholm膝关节评分;比较2组治疗前后C反应蛋白(CRP)、红细胞沉降率(ESR)水平;比较2组治疗前后膝关节周径。结果治疗组总有效率91.30%(42/46),对照组总有效率73.91%(34/46),治疗组疗效优于对照组(P<0.05)。2组治疗后疼痛VAS均较本组治疗前降低(P<0.05),Lysholm膝关节评分较本组治疗前升高(P<0.05);治疗后治疗组疼痛VAS低于对照组(P<0.05),Lysholm膝关节评分高于对照组(P<0.05)。2组治疗后CRP、ESR均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后膝关节周径均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。结论着痹方联合中药溻渍治疗痰饮流注型膝关节滑膜炎,可有效减轻患者疼痛程度,改善膝关节活动功能,缓解膝关节肿胀,降低炎症因子水平,有利于膝关节滑膜炎患者康复。Objective To observe the clinical effect of the combination of Zhuobi Formula and Chinese medicine drench-soak therapy on gonarthromeningitis of phlegm dampness and collateral abstruction.Methods Totally 92 patients with gonarthromeningitis of phlegm dampness and collateral abstruction were randomly assinged into control group(n=46)and treatment group(n=46).All patients were given Etoricoxib Tablets,the patients of treatment group were additionally given Zhuobi Formula and Chinese medicine drench-soak therapy.The treatment was performed for two weeks,aiming to compare the curative effect,the scores of visual analogue scale(VAS)and Lysholm knee,and the levels of C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR)between groups before and after treatment.The circumference of knee joint was assessed.Results The total effective rate in the treatment group was better than that of the control group(91.30%[42/46]vs 73.91%[34/46],P<0.05).Significantly reduced VAS pain scores(P<0.05)and significantly increased Lysholm knee scores(P<0.05)were detected in both groups,and the treatment group was superior to the control group for pain VAS and Lysholm knee score(both P<0.05).After treatment,the levels of CRP and ESR in the two groups were significantly lower(P<0.05),and the decrease in the treatment group was more significant(P<0.05).After treatment,the circumference of knee joint was significantly shorter(P<0.05),and which in the treatment group was more common(P<0.05).Conclusion For gonarthromeningitis of phlegm dampness and collateral abstruction,the combination of Zhuobi Formula and and Chinese medicine drench-soak therapy can effectively reduce the degree of pain,improve the function of knee joint movement,relieve knee joint swelling,and reduce the level of inflammatory factors,which is a therapeutic means conducive to their rehabilitation.
