64排螺旋CT冠状动脉成像诊断老年冠状动脉狭窄的应用价值

摘　　要：目的:探讨64排螺旋CT冠状动脉(以下简称冠脉)成像诊断老年冠状动脉狭窄的应用价值。方法:选择2020年1月—2023年6月收治的老年冠心病患者50例,均接受64排128层螺旋CT(64-SCTA)及冠状动脉造影(CAG)检查,以CAG检查结果作为冠脉狭窄诊断的“金标准”,计算64-SCTA检查的诊断价值指标,包括灵敏度、特异度和准确度。结果:64-SCTA诊断冠状动脉狭窄的灵敏度、特异度及准确度分别为64.82%,94.35%和70.80%;进一步去除钙化节段,64-SCTA诊断冠状动脉狭窄的灵敏度、特异度及准确度分别为84.09%,97.33%和84.09%,其中诊断灵敏度和准确度较前显著提高,差异有统计学意义(P<0.05),特异度较前提高,但是差异无统计学意义(P>0.05)。结论:64排螺旋CT冠状动脉成像检查诊断老年冠状动脉狭窄的灵敏度和准确度较高。Objective:To explore the application value of 64 slice spiral CT coronary angiography in the diagnosis of elderly coronary artery stenosis.Methods:Fifty elderly patients with coronary heart disease treated io our hospital from January 2020 to June 2023 were selected as the study subjects,all of whom underwent 64 slice 128 slice spiral CT(64-SCTA)and coronary angiography(CAG)examinations.The results of CAG examination were used as the'gold standard'for the diagnosis of coronary artery stenosis,and the diagnostic value indicators of 64-SCTA examination were calculated,including sensitivity,specificity,and accuracy.Results:The sensitivity,specificity,and accuracy of 64-SCTA in the diagnosis of coronary artery stenosis were 64.82%,94.35%,and 70.80%,respectively.Furthermore,for the removal of calcified segments,the sensitivity,specificity,and accuracy of 64-SCTA in the diagnosis of coronary artery stenosis were 84.09%,97.33%,and 84.09%,respectively.The diagnostic sensitivity and accuracy were significantly improved compared to before,with a significant difference(P<0.05).The specificity was improved compared to before,but the difference was not significant(P>0.05).Conclusion:64 slice spiral CT coronary angiography has high sensitivity and accuracy in diagnosing elderly coronary artery stenosis,and has high practical value.
