肱骨髓内钉与肱骨近端锁定板治疗肱骨近端骨折的治疗效果比较

摘　　要：目的:探讨使用肱骨髓内钉与肱骨近端锁定(PHILOS)板治疗肱骨近端骨折的效果。方法:选取2019年8月—2022年6月四川省江油市第二人民医院骨科采用手术治疗的肱骨近端骨折患者83例,根据手术方式的不同分为肱骨髓内钉手术组(n=38)与PHILOS板手术组(n=45)。结果:本研究结果显示,肱骨髓内钉手术组出血量少于PHILOS板手术组,手术时间、住院时间短于PHILOS板手术组,差异有统计学意义(P<0.05)。髓内钉手术组患者的手术优良率为92.1%,高于PHILOS板手术组的84.44%,差异无统计学意义(P>0.05)。术后3个月、6个月、1年,两组骨折愈合情况比较,差异无统计学意义(P>0.05)。结论:肱骨髓内钉手术治疗肱骨近端骨折与PHILOS板手术的疗效基本一致,且手术时间、出血量、住院时间均少于PHILOS锁定板手术,且肱骨髓内钉手术术中一般无需植骨,对肱骨近端复杂骨折、多段骨折、长斜形骨折的治疗效果更好。Objective To explore the efficacy of using humeral intramedullary nail system compared to the proximal humeral internal locking system(PHILOS)plate in the treatment of proximal humeral fractures.Methods A total of 83 patients with proximal humeral fractures who underwent surgical treatment in the Department of Orthopedics at the Second People's Hospital of Jiangyou City,Sichuan Province,from August 2019 to June 2022 were selected.Patients were divided into two groups based on the different surgical methods:the humeral intramedullary nail surgery group(n=38)and the PHILOS plate surgery group(n=45).Results The results of this study showed that the blood loss in the intramedullary nail surgery group was less than that in the PHILOS plate surgery group,and the operation time and hospital stay were shorter than those in the PHILOS plate surgery group,with statistically significant differences(P<0.05).The excellent rate of surgery in the intramedullary nail surgery group was 92.1%,which was higher than that in the PHILOS plate surgery group(84.44%),but the difference was statisticallynon significant(P>0.05).At 3 months,6 months,and 1 year postoperatively,there was statisticallynon significant difference in fracture healing between the two groups(P>0.05).Conclusions The efficacy of humeral intramedullary nail system in the treatment of proximal humeral fractures is basically consistent with traditional PHILOS plate surgery,and the operation time,blood loss,and hospital stay are less than those in the PHILOS plate surgery,generally not requiring bone grafting during surgery.It provides better treatment effects for complex proximal humeral fractures,multi-segment fractures,and long oblique fractures of the humerus.
