某院静脉用药调配中心全肠外营养液医嘱合理性分析

摘　　要：目的分析某院静脉用药调配中心全肠外营养液医嘱的合理性。方法回顾性分析医院2022年1月至2023年1月收治的120例开展静脉用药调配中心全肠外营养液治疗患者的临床资料,参考相关指南、共识分析静脉用药调配中心全肠外营养液医嘱的合理性,总结预防对策。结果120例全肠外营养液治疗患者中,不合理医嘱共13例,不合理医嘱占比10.83%,包括热氮比不合理5例,氨基酸浓度不合理3例,二价阳离子不合理、葡萄糖浓度不合理各2例,组方不合理1例。结论该院静脉用药调配中心全肠外营养液不合理医嘱类型相对较多,需要加强医嘱审核,并通过对应措施进行预防处理,以保证医嘱合理、有效、安全。Objective To analyze the rationality of the prescription for total parenteral nutrition solution in the intravenous medication dispensing center of a hospital.Methods A total of 120 patients underwent total parenteral nutrition treatment in the PIVAS from January 2022 to January 2023 were analyzed retrospectively.The relevant guidelines and consensus analysis were referred,the rationality of the total parenteral nutrition prescription at the intravenous drug dispensing center was analyzed,and preventive measures were summarized.Results Among the 120 patients treated with total parenteral nutrition solution,there were a total of thirteen cases with irrational medical orders,accounted for 10.83%,including five cases of unreasonable heat to nitrogen ratio,three cases of unreasonable amino acid concentration,two cases of unreasonable divalent cation and glucose concentration,and one case of unreasonable formula.Conclusion There are relatively many types of irrational medical orders for total parenteral nutrition solution in the intravenous medication dispensing center of the hospital.It is necessary to strengthen the review of medical orders and take corresponding measures for prevention and treatment to ensure that medical orders are reasonable,effective,and safe.
