血清α-L-岩藻糖苷酶、甲胎蛋白、异常凝血酶原检测在原发性肝癌诊断中的价值研究

摘　　要：目的分析在诊断原发性肝癌患者时采取血清α-L-岩藻糖苷酶(α-l-fucosidase,AFU)、甲胎蛋白(α-fetoprotein,AFP)、异常凝血酶原Ⅱ(Protein Induced by Vitamin K Absence or Antagonist-Ⅱ,PIVKA-Ⅱ)检测的价值。方法选取2022年8月—2023年8月邳州市中医院收治的83例原发性肝癌患者以及肝硬化患者作为研究对象,依据疾病类型不同,分为良性组(肝硬化患者41例)、观察组(原发性肝癌患者42例),另选择83例健康人群作为对照组。检测3组血清AFU、AFP、PIVKA-Ⅱ指标,分析上述指标水平,同时通过ROC曲线分析单项以及联合检测诊断原发性肝癌的价值。结果两组患者AFU、AFP、PIVKA-Ⅱ指标均显著高于对照组,且观察组AFU、AFP、PIVKA-Ⅱ指标水平高于良性组,差异有统计学意义(P均<0.05)。联合检测时,AUC提升至0.912,灵敏度提升至89.63%,特异度提升至99.10%,高于单独检测。结论在诊断原发性肝癌患者中,采用联合AFU、AFP、PIVKA-Ⅱ检测方式,有较为理想的诊断价值,能够为完善治疗方案奠定基础,可以作为诊断原发性肝癌的有效方法之一。Objective To analyze the diagnostic value of serumα-L-fucosidase(AFU),α-fetoprotein(AFP),protein induced by vitamin K absence or antagonist-Ⅱ(PIVKA-Ⅱ)in patients with primary liver cancer.Methods A total of 83 patients with primary liver cancer and liver cirrhosis admitted to Pizhou Hospital of Traditional Chinese Medicine from August 2022 to August 2023 were selected as the study subjects.According to different disease types,they were divided into benign group(41 patients with liver cirrhosis),observation group(42 patients with primary liver cancer),and 83 healthy people as control group.Serum AFU,AFP and PIVKA-Ⅱindexes were detected in the 3 groups,and the levels of the above indexes were analyzed,and the value of single and combined detection in the diagnosis of pri⁃mary liver cancer was analyzed by ROC curve.Results AFU,AFP and PIVKA-Ⅱindexes in two groups were signifi⁃cantly higher than those in control group,and the levels of AFU,AFP and PIVKA-Ⅱindexes in observation group were higher than those in benign group,and the differences were statistically significan(all P<0.05).In the combined detection,the area under curve(AUC)was increased to 0.912,the sensitivity was increased to 89.63%,and the speci⁃ficity was increased to 99.10%,which was higher than that of the single detection.Conclusion In the diagnosis of pa⁃tients with primary liver cancer,the combination of AFU,AFP and PIVKA-Ⅱdetection has ideal diagnostic value,which can lay a foundation for the improvement of treatment plan,and can be used as one of the effective methods for the diagnosis of primary liver cancer.
