血府逐瘀汤联合针灸方案在冠心病不稳定性心绞痛患者治疗中的应用效果

摘　　要：目的 观察血府逐瘀汤联合针灸方案在冠心病不稳定性心绞痛患者治疗中的应用效果。方法 选取2020年12月—2022年4月济南市中西医结合医院收治的冠心病不稳定性心绞痛患者107例,采用随机数字表法分为试验组54例和对照组53例。对照组给予常规西药治疗,试验组给予血府逐瘀汤联合针灸治疗,2组患者均连续治疗30 d。比较2组治疗效果,治疗前后心功能相关指标[左室射血分数(LVEF)、每搏心输出量(SV)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)]、视觉模拟评分(VAS)、心绞痛持续时间及发作次数、不良反应。结果 试验组治疗总有效率为98.15%,高于对照组的84.91%(χ^(2)=4.491,P=0.034)。治疗30 d后,2组LVEF、SV高于治疗前,LVESD、LVEDD小于治疗前,且试验组变化幅度大于对照组(P<0.01);2组VAS评分、心绞痛发作次数均低于治疗前,心绞痛持续时间短于治疗前,且试验组低、短于对照组(P<0.01)。试验组不良反应总发生率为3.70%,低于对照组的16.98%(χ^(2)=5.112,P=0.023)。结论 血府逐瘀汤联合针灸方案治疗冠心病不稳定性心绞痛效果较好,能有效改善心功能指标,改善心绞痛症状,降低不良反应发生率。Objective To observe the effect of Xuefu Zhuyu decoction combined with acupuncture scheme in the treatment of patients with unstable angina pectoris of coronary heart disease.Methods A total of 107 patients with unstable angina pectoris of coronary heart disease treated in Jinan Hospital of Integrated Traditional Chinese and Western Medicine from December 2020 to April 2022 were selected and divided into test group(54 cases)and control group(53 cases)by random number table method.The control group was treated with conventional western medicine,and the test group was treated with Xuefu Zhuyu decoction combined with acupuncture.Both groups were treated continuously for 30 days.The therapeutic effect,the cardiac function related indexes(LVEF,SV,LVESD,LVEDD),VAS,duration and frequency of angina pectoris before and after treatment,and adverse reactions were compared between the two groups.Results The total effective rate of the test group was 98.15%,which was higher than 84.91%of the control group(χ^(2)=4.491,P=0.034).After 30 days of treatment,LVEF and SV in two groups were higher than before treatment,LVESD and LVEDD were lower than before treatment,and the change range of test group were greater than that of control group(P<0.01).VAS and frequency of angina pectoris in two groups were lower than before treatment,and the duration of angina pectoris were shorter than before treatment,and the test group were lower than control group(P<0.01).The total incidence of adverse reactions in test group was 3.70%,which was lower than 16.98%of control group(χ^(2)=5.112,P=0.023).Conclusion Xuefu Zhuyu decoction combined with acupuncture scheme is effective in the treatment of unstable angina pectoris of coronary heart disease,which can effectively improve the index of heart function,improve the symptoms of angina pectoris and reduce the incidence of adverse reactions.
