摘 要:目的采用Meta分析评价养血清脑颗粒治疗慢性脑供血不足(CCCI)的有效性和安全性。方法通过计算机检索中国知网(CNKI)、万方数据知识服务平台、维普网数据库(VIP)、PubMed等数据库,选取自建库至2023年1月公开发表的随机对照临床试验,应用RevMan 5.4软件对养血清脑颗粒治疗CCCI的有效性和安全性进行Meta分析。结果共纳入研究17项,涉及1656例患者。试验组总有效率高于对照组[OR=4.19,95%CI(3.14~5.58),P<0.00001];试验组血浆纤维蛋白原含量低于对照组[MD=0.96,95%CI(0.67~1.26),P<0.00001];采用养血清脑颗粒治疗CCCI的患者椎动脉平均血流速度大于单纯西药治疗CCCI的患者[MD=6.58,95%CI(2.08~11.09),P=0.004];试验组基底动脉平均血流速度大于对照组[MD=3.91,95%CI(2.09~5.74),P<0.0001]。试验组与对照组不良反应轻微且比较差异无统计学意义[OR=1.06,95%CI(0.51,2.24),P=0.87]。总有效率漏斗图呈不完全对称分布,提示可能存在发表偏倚。结论养血清脑颗粒可提高CCCI治疗总有效率,有效改善血浆纤维蛋白原含量与椎动脉、基底动脉平均血流速度。Objective Meta-analysis was used to evaluate the efficacy and safety of Yangxue Qingnao granule in the treatment of chronic cerebral circulation insufficiency(CCCI).Methods Through computer search of CNKI,Wanfang Data knowledge service platform,VIP,PubMed and other databases,the randomized controlled clinical trials published from the establishment of the database to January 2023 were selected,and Revman 5.4 software was used to Meta-analysis the efficacy and safety of Yangxue Qingnao granule in the treatment of CCCI.Results A total of 17 studies involving 1656 patients were included.The total effective rate of the experimental group was higher than that of the control group[OR=4.19,95%CI(3.14~5.58),P<0.00001].The plasma fibrinogen content in the experimental group was lower than that in the control group[MD=0.96,95%CI(0.67~1.26),P<00001].The average blood flow velocity of vertebral artery in patients with CCCI treated with Yangxue Qingnao granule was higher than that in patients with CCCI treated with western medicine alone[MD=6.58,95%CI(2.08~11.09),P=0.004].The average blood flow velocity of basilar artery in the experimental group was higher than that in the control group[MD=3.91,95%CI(2.09~5.74),P<0.0001].The adverse reactions of the experimental group and the control group were mild,and there was no significant difference between the two groups[OR=1.06,95%CI(0.51~2.24),P=0.87].The funnel plot of total effective rate showed an incomplete symmetrical distribution,suggesting that there may be publication bias.Conclusion Yangxue Qingnao granule can improve the total effective rate of CCCI treatment,effectively improve the plasma fibrinogen content and the average blood flow velocity of vertebral artery and basilar artery.
