摘 要:目的 分析CYP3A7、MDR1基因多态性与镇痛效应之间的关系。方法 回顾性分析87例行腹部手术患者的一般临床资料,术后均采用舒芬太尼镇痛,对影响患者术后镇痛效应的相关因素进行分析。结果 87例行腹部手术的患者中,术后轻度疼痛(轻度疼痛组)45例、术后中重度疼痛(中重度疼痛组)42例。两组的年龄、性别、美国麻醉医师协会(ASA)分级以及CYP3A7基因多态性比较差异无统计学意义(P>0.05);两组MDR1 2677G>T/A基因多态性比较差异有统计学意义(P<0.05)。将轻度疼痛组与中重度疼痛组患者具有统计学意义的单因素纳入Logistic回归模型进行多因素分析,结果表明MDR1 2677G>T/A GG、GA、TA、AA为术后镇痛效应的影响因素(OR=0.998、1.078、1.044、1.038,P<0.05)。结论 MDR1基因多态性对患者术后镇痛效应可产生影响,临床在制定镇痛方案时应该综合考虑患者基因型等相关因素制定个体化的镇痛治疗方案。Objective To analyze the correlation of polymorphism of CYP3A7 and MDR1 genes and analgesic effect.Methods The general clinical data of 87 patients undergoing abdominal surgery were retrospectively analyzed.Sufentanil was used for postoperative analgesia,and the related factors affecting the postoperative analgesia effect of patients were analyzed.Results Among the 87 patients who underwent abdominal surgery,there were 45 cases of mild postoperative pain(mild pain group)and 42 cases of moderate to severe postoperative pain(moderate to severe pain group).There was no significant difference in age,gender,American Society of Anesthesiologists(ASA)grade and CYP3A7 gene polymorphism between the mild pain group and the moderate to severe pain group(P>0.05).The difference between the two groups was statistically significant when comparing the polymorphisms of the MDR12677G>T/A gene(P<0.05).The statistically significant single factors of patients in the mild pain group and the moderate to severe pain group were included in the Logistic regression model for multifactorial analysis,and the results showed that MDR12677G>T/A GG,GA,TA,and AA were the factors affecting postoperative analgesic effect(OR=0.998,1.078,1.044,1.038;P<0.05).Conclusion MDR1 gene polymorphism may affect the postoperative analgesic effect of patients,and the relevant factors such as patient genotype should be comprehensively considered in the formulation of clinical analgesic treatment plan.
