从脾论治糖尿病肌少症的中医药研究进展

摘　　要：肌少症是糖尿病的慢性并发症之一,可严重降低患者生活质量,加重医疗及社会负担。中医学所言之“脾”与糖尿病肌少症密切相关,通过古今诸多论述与研究的梳理,总结得出禀赋不足、饮食不节、劳欲过度等病因导致的“脾失健运、肌肉失养”为糖尿病肌少症的核心病机。因此,以“益气健脾”为主要治则治法,通过健脾类单味中药(黄芪、人参、当归等)、中药复方(四君子汤、八珍汤、补中益气汤等)、针刺推拿及功法治疗的运用,为未来中医药继续发挥整体调控优势从脾论治糖尿病肌少症提供了新思路。As one of the chronic complications of diabetes mellitus,sarcopenia seriously reduces the life quality of the patients and increases the medical and social burden.The"spleen"in traditional Chinese medicine(TCM)is inextricably linked to diabetic sarcopenia.Through sorting a number of ancient and modern discussions and studies,it′s concluded that the core pathogenesis of diabetic sarcopenia is the dysfunction of spleen in transport and muscle dystrophy due to a variety of causes,including constitutional insufficiency,improper diet and excessive exertion.Therefore,the main treatment method is to"replenishing qi to invigorate the spleen".The use of pi-invigorating single herbs(Astragalus membranaceus,ginseng,Angelica sinensis,etc.),herbal compound prescriptions(Sijunzi decoction,Bazhen decoction,Buzhong Yiqi decoction,etc.),acupuncture,massage and Gongfa treatment provide new ideas for continuously exerting the TCM advantages of overall regulation and treatment of diabetic sarcopenia based on the spleen theory in the future.
