摘　　要：目的 探讨胎头着冠后口令式深呼吸法应用于减少会阴撕裂的效果。方法 选取2023年1—12月上海市东方医院吉安医院妇产科收治的100例产妇作为研究对象,通过单双号随机分组法分为观察组(n=50)与对照组(n=50)。对照组予以常规产房护理,观察组采用胎头着冠后口令式深呼吸法。比较两组产妇会阴裂伤程度、第二产程时间、产后出血量,比较两组新生儿1 min、5 min时Apgar评分,统计两组产妇并发症发生情况及护理满意度。结果 观察组Ⅱ度会阴裂伤患者比例显著低于对照组,未撕裂比例明显高于对照组,差异有统计学意义(P<0.05);观察组第二产程时间明显短于对照组,产后出血量显著少于对照组,差异有统计学意义(P<0.05);观察组新生儿1 min、5 min时Apgar评分明显高于对照组(P<0.05);观察组产妇产后并发症发生率明显低于对照组(P<0.05);观察组产妇护理满意度显著高于对照组(P<0.05)。结论 胎头着冠后口令式深呼吸法能显著改善产妇在自然分娩中会阴撕裂程度,缩短产程时间,降低并发症发生风险,新生儿整体呼吸情况良好,产妇满意度较高。Objective To explore the effect of watchful deep breathing after fetal head crown on reducing perineal tear.Methods A total of 100 parturients admitted to the January and December Department of Ji 'an Hospital of Shanghai Dongfang Hospital from April to August 2023 were selected as the study objects and divided into observation group(n=50) and control group(n=50) by odd-even random grouping method.The control group was given routine nursing care in the delivery room,and the observation group was given watchful deep breathing after fetal head crown.The degree of perineal laceration,the time of the second stage of labor and the amount of postpartum bleeding were compared between the two groups,and the Apgar score at 1 min and 5 min between the two groups were compared.The incidence of complications and nursing satisfaction of the two groups were analyzed.Results The proportion of patients with degree Ⅱ perineal laceration in observation group was significantly lower than that in control group,and the proportion without laceration was significantly higher than that in control group,the difference was statistically significant(P<0.05).The second stage of labor in the observation group was significantly shorter than that in the control group,and the postpartum blood loss was significantly less than that in the control group,the difference was statistically significant(P<0.05).Apgar score in observation group was significantly higher than that in control group at 1 min and 5 min(P<0.05).The incidence of postpartum complications in the observation group was significantly lower than that in the control group(P<0.05).The satisfaction of maternal nursing in observation group was significantly higher than that in control group(P<0.05).Conclusion The command deep breathing method after the fetal head crown can significantly improve the degree of perineal tear in natural delivery,shorten the labor time,reduce the risk of complications,the overall breathing of the newborn is good,and the maternal satisfaction is high.
