心脏彩超与心电图诊断高血压性心脏病的效果对比

摘　　要：目的对比研究在高血压性心脏病诊断中心脏彩超与心电图的效果。方法抽选93例本院2022年7月至2023年7月收治的疑似高血压性心脏病患者为研究对象,所有患者分别进行心脏彩超检查和心电图检查,对比检查结果,并以超声心动图最终的结果当作诊断金标准分析诊断效能。结果心脏彩超诊断主动脉弹性降低阳性率为17.20%、高于心电图的7.53%,左室扩大阳性率为24.73%、高于心电图的12.90%,左房增大阳性率为26.88%、高于心电图的13.98%,左室后壁肥厚阳性率为16.13%、高于心电图的6.45%,主动脉扩张的阳性率为30.11%、高于心电图的15.05%,左室肥厚的阳性率为45.16%、高于心电图的27.96%,组间差异呈统计学意义(P<0.05)。心脏彩超诊断高血压性心脏病的阳性率为94.62%,高于心电图的86.02%,组间差异呈统计学意义(P<0.05)。心脏彩超诊断准确度为97.85%、高于心电图的87.10%,诊断敏感度为97.78%、高于心电图的87.78%,诊断特异度为100.00%、高于心电图的66.67%,诊断阳性预测值为100.00%、高于心电图的98.75%,阴性预测值为60.00%、高于心电图的15.38%。结论在高血压性心脏病临床诊断中,心脏彩超的诊断效果优于心电图,而且有着更高的诊断效能。Objective To compare the effect of color Doppler ultrasound and electrocardiogram in the diagnosis of hypertensive heart disease.Methods Ninety-three patients with suspected hypertensive heart disease admitted to our hospital from July 2022 to July 2023 were selected as the study objects.All patients were examined by color Doppler ultrasonography and electrocardiogram respectively,and the results were compared with echocardiography results to analyze the diagnostic efficiency.Results The positive rate of aortic elasticity reduction was 17.20%,higher than that of electrocardiogram(7.53%),the positive rate of left ventricular enlargement was 24.73%,higher than that of electrocardiogram(12.90%),and the positive rate of left atrial enlargement was 26.88%,higher than that of electrocardiogram(13.98%).The positive rate of left ventricular posterior wall hypertrophy was 16.13%,higher than that of electrocardiogram(6.45%),the positive rate of aortic dilation was 30.11%,higher than that of electrocardiogram(15.05%),and the positive rate of left ventricular hypertrophy was 45.16%,higher than that of electrocardiogram(27.96%),with statistical significance between groups(P<0.05).The positive rate of diagnosis of hypertensive heart disease by color Doppler ultrasound was 94.62%,higher than that by ECG(86.02%),and the difference between groups was statistically significant(P<0.05).The diagnostic accuracy of color Doppler ultrasound was 97.85%,higher than 87.10%of ECG,diagnostic sensitivity was 97.78%,higher than 87.78%of ECG,diagnostic specificity was 100.00%,higher than 66.67%of ECG,diagnostic positive predictive value was 100.00%,higher than 98.75%of ECG.The negative predictive value was 60.00%,higher than 15.38%in ECG.Conclusion In the clinical diagnosis of hypertensive heart disease,the diagnostic effect of cardiac color ultrasound is better than that of electrocardiogram,and it has higher diagnostic efficiency.
