通腑宣肺汤加减联合思维导图引导优质护理在支原体肺炎患儿中的临床获益

摘　　要：目的探讨小儿支原体肺炎(MP)接受通腑宣肺汤加减联合思维导图引导优质护理的临床效果。方法本次研究共纳入研究对象47例,此次研究对象为郑州市中医院选取的MP患儿,于2021年2月至2023年1月完成选例,以随机数字表法为分组原则将患儿分配成两组,两组患儿均给予思维导图引导下的优质护理,其中一组23例以阿奇霉素序贯治疗,确定为A组;另一组24例以通腑宣肺汤加减治疗,确定为B组。两组均治疗21d。比较两组症状、体征消失时间(治疗后)、中医证候积分、肺功能、炎性因子(治疗前以及治疗21 d后)各项指标。结果A、B组各项症状、体征消失时间比较,B各项症状、体征消失时间低于A组(P<0.05);治疗21 d后与治疗前比,A、B组患儿气促喘憋、喉间痰鸣、咳嗽无力、恶寒发热各项中医证候积分、RR水平以及血清IL-6、IL-8、MCP-4水平均降低,且B组低于A组(P<0.05);治疗21d后与治疗前比,A、B组患儿MMF、PEF水平均升高,且B组高于A组(P<0.05);治疗21 d后与治疗前比,A、B组患儿FEV1水平以及血清IL-4水平均升高,且B组高于A组(P<0.05)。结论通腑宣肺汤加减治疗联合思维导图引导优质护理可促进MP患儿的症状、体征消失,改善中医证候,促进肺功能恢复,降低炎性因子水平。Objective To explore the clinical effect of treating children with mycoplasma pneumonia(MP)with modified Tongfu Xuanfei decoction combined with mind mapping to guide quality nursing.Methods A total of 47 subjects were included in this study.The subjects were MP children selected by Zhengzhou T.C.M Hospital.The selected cases were completed from February 2021 to January 2023,and the children were divided into two groups based on the random number table method,and both groups were given high-quality care guided by mind mapping,one group of 23 patients was treated with azithromycin successively and was identified as group A;The other group of 24 patients were treated with Tongfu Xuanfei decoction and identified as group B.Both groups were treated for 21 days.Symptom and sign disappearance time(after treatment),TCM syndrome score,lung function,inflammatory factors(before treatment and 21 d after treatment)were compared between the two groups.Results Comparison of the disappearance time of symptoms and signs in group A and B showed that the disappearance time of symptoms and signs in group B was lower than that in group A(P<0.05).After 21 days of treatment,compared with before treatment,the scores of TCM syndromes,RR levels and serum IL-6,IL-8 and MCP-4 levels of children in the group A and the group B were lower,group B was lower than group A(P<0.05).After 21 days of treatment,the MMF and PEF of children in the group A and the group B were higher than those before treatment,group B was higher than group A(P<0.05).After 21 days of treatment,the FEV1 level and serum IL-4 level of children in the group A and the group B were higher than those before treatment,group B was higher than group A.(P<0.05).Conclusions Modified Tongfu Xuanfei decoction combined with mind mapping to guide high-quality nursing can promote the disappearance of symptoms and signs of MP children,improve TCM syndromes,promote the recovery of lung function and reduce the level of inflammatory factors.
