摘  要:目的 探讨肺康复治疗联合无创机械通气对慢性阻塞性肺疾病(COPD)患者肺功能及日常生活活动能力的影响。方法 82例COPD患者,按随机数字表法分为对照组与观察组,各41例。对照组给予无创机械通气,观察组在对照组基础上加以肺康复治疗。对比两组临床疗效、肺功能、日常生活活动能力。结果 观察组治疗总有效率92.68%高于对照组的75.61%,差异有统计学意义(P<0.05)。治疗前,两组呼气流量峰值(PEF)、第1秒用力呼气容积(FEV1)、用力肺活量(FVC)对比,差异无统计学意义(P>0.05);治疗1周后,两组PEF、FEV1、FVC均高于本组治疗前,且观察组PEF(4.21±0.71)L/s、FEV1(1.97±0.45)L、FVC(2.78±0.53)L均高于对照组的(3.59±0.63)L/s、(1.53±0.36)L、(2.35±0.45)L,差异有统计学意义(P<0.05)。治疗前,两组日常生活活动能力量表(ADL)评分对比,差异无统计学意义(P>0.05);治疗1周后,两组ADL评分均高于本组治疗前,且观察组ADL评分(80.75±4.23)分高于对照组的(70.69±3.51)分,差异有统计学意义(P<0.05)。结论 肺康复治疗+无创机械通气能够有效改善COPD患者的肺功能,提高其日常生活活动能力,值得临床的大力推行。Objective To discuss the effects of pulmonary rehabilitation therapy combined with noninvasive mechanical ventilation on pulmonary function and activities of daily living in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 82 COPD patients were divided into a control group and an observation group according to random numerical table,with 41 cases in each group.The control group was given non-invasive mechanical ventilation,and the observation group was given pulmonary rehabilitation treatment on the basis of the control group.The clinical efficacy,pulmonary function and activities of daily living were compared between the two groups.Results The total effective rate of 92.68%in the observation group was higher than that of 75.61%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference between the two groups in terms of peak expiratory flow(PEF),forced expiratory volume in one second(FEV1)and forced vital capacity(FVC)(P>0.05).After 1 week of treatment,PEF,FEV1 and FVC in both groups were higher than those before treatment in this group;the observation group had PEF of(4.21±0.71)L/s,FEV1 of(1.97±0.45)L and FVC of(2.78±0.53)L,which were higher than those of(3.59±0.63)L/s,(1.53±0.36)L and(2.35±0.45)L in the control group;the differences were statistically significant(P<0.05).Before treatment,there was no statistically significant difference in activities of daily living(ADL)scores between the two groups(P>0.05).After 1 week of treatment,ADL score in both groups was higher than that before treatment in this group,and ADL score of(80.75±4.23)points in the observation group was higher than that of(70.69±3.51)points in the control group.The differences were statistically significant(P<0.05).Conclusion Pulmonary rehabilitation therapy+non-invasive mechanical ventilation can effectively improve the pulmonary function of patients with COPD and improve their activities of daily living,which is worthy of
