摘　　要：免疫性血小板减少症(ITP)是一种以血小板生成减少和破坏增多为特征的获得性自身免疫性疾病,现普遍认为其发病与体液免疫、细胞免疫、遗传易感性、基因表达异常、感染等多种因素有关。幽门螺杆菌(Hp)在人群中易感,与胃炎、消化性溃疡等消化道疾病密切相关,最近研究发现Hp也可能与血液系统疾病ITP的发生相关。Hp诱发ITP的机制涉及Hp毒力因子、单核细胞吞噬功能增加、诱导血小板聚集与活化、树突状细胞与辅助性T淋巴细胞平衡紊乱、地域与遗传因素等多个方面。本文主要就Hp感染诱发ITP的发病机制相关研究进展作一综述,为ITP的治疗提供新的研究思路。Immune thrombocytopenia(ITP)is an acquired autoimmune disease characterized by decreased platelet production and increased destruction.It is widely believed that its onset is related to various factors such as humoral immunity,cellular immunity,genetic susceptibility,abnormal gene expression,and infection.Helicobacter pylori(Hp)is susceptible in the population,and is closely related to gastrointestinal diseases such as gastritis and peptic ulcers.Recent studies have found that Hp may also be associated with the occurrence of hematological disease,such as ITP.The mechanism of Hp induced ITP involves multiple aspects,including Hp virulence factors,increased monocyte phagocytic function,induction of platelet aggregation and activation,imbalance of dendritic cells and T helper lymphocytes,and regional and genetic factors.This article mainly reviewed the research progress on the pathogenesis of ITP induced by Hp infection,providing new research ideas for the treatment of ITP.
