摘　　要：目的探讨MRI多序列成像在肛周脓肿、肛瘘的诊断及分型中的应用。方法选取2017年12月至2021年3月武穴市第一人民医院收治的60例肛周脓肿、肛瘘患者作为研究对象,所有患者均经MRI检查和手术病理诊断。统计肛周脓肿分型及肛瘘分型,记录肛瘘支管、主管、内口、脓腔检出情况。结果MRI诊断肛周脓肿分型中2例坐骨直肠窝脓肿误诊为直肠黏膜下脓肿,MRI检查诊断符合率为95.00%;MRI检查漏诊支管1例、主管1例、内口1例,MRI诊断符合率为85.00%;RI诊断肛瘘分型诊断中2例括约肌间型误诊为复杂型,MRI检查诊断符合率为90.00%。结论MRI多序列成像可准确分型和诊断肛周脓肿、肛瘘,误诊和漏诊相对较低,值得临床推广应用。Objective To investigate the application of MRI multi-sequence imaging in the diagnosis and classification of perianal abscess and anal fistula.Methods 60 patients with perianal abscess and anal fistula admitted to the First People's Hospital ofWuxue City from December 2017 to March 2021 were selected as the study objects.All patients were diagnosed by MRI examination and surgical pathological diagnosis.The types of perianal abscesses and anal fistulas were analyzed,and the detection of branches,main pipes,inner orifices and pus cavities in anal fistula were recorded.Results Among the types of perianal abscess diagnosed by MRI,2 cases of ischiorectal abscess were misdiagnosed as submucous rectal abscess,and the diagnosis coincidence rate of MRI was 95.00%.MRI missed diagnosis branch canal in 1 case,main canal in 1 case and internal orifice in 1 case,with MRI diagnosis coincidence rate was 85.00%.In MRI diagnosis of anal fistula classification,2 cases of intersphincteric type were misdiagnosed as complex type,and the diagnosis coincidence rate of MRI was 90.00%.Conclusion MRI multi-sequence imaging can accurately classify and diagnose perianal abscess and anal fistula,with relatively low misdiagnosis and missed diagnosis,which is worthy of clinical promotion and application.
