肌内注射腺苷钴胺、威伐光理疗联合梅花针在急性期带状疱疹中的应用

摘　　要：目的探讨肌内注射腺苷钴胺、威伐光理疗联合中医梅花针对急性期带状疱疹治疗的有效性、优越性。方法选取皮肤科门诊2020年9月至2022年5月确诊的124例带状疱疹患者,患者随机分为对照组58例和观察组66例。对照组采用西医治疗,即肌内注射腺苷钴胺、口服伐昔洛韦及维生素B1、辅助威伐光理疗。观察组在对照组基础上,联合应用梅花针针刺、火罐治疗。比较2组患者的治疗效果、疼痛缓解时间、以及是否遗留后遗神经痛的差异,综合评价观察组的临床效果。结果2组患者年龄、病程差异无统计学意义(P>0.05)。观察组疗效明显优于对照组(χ^(2)=8.904,P=0.031)。观察组疼痛缓解时间明显短于对照组(t=3.402,P=0.001)。观察组出现后遗神经痛概率明显低于对照组(χ^(2)=4.835,P=0.028)。结论肌内注射腺苷钴胺、威伐光理疗联合梅花针治疗急性期带状疱疹,能够提高治疗的有效率,明显缩短疼痛缓解时间,有效的减少了后遗神经痛的发生率。Objective To explore the effectiveness and superiority of intramuscular injection of Cobamamide,water-filtered infrared A(wIRA)irradiation combined with plum-blossom needle(PBN)for acute herpes zoster(AHZ).Methods From September 2020 to May 2022,totally 124 AHZ patients diagnosed in the Department of Dermatology of our hospital were randomly divided into the control group and the observation group.All patients were managed by Western medicine treatment(intramuscular injection of Cobamamide,oral valacyclovir and vitamin B1,supplemented with wIRA irradiation),and those in the observation group were additionally treated with PBN acupuncture and fire cupping therapy.The therapeutic outcomes,pain relief time,and the incidence of postherpetic neuralgia were compared between groups.The clinical effectiveness in the observation group was comprehensively assessed.Results The difference was not statistically significant in age and course of disease between groups(P>0.05).The curative effect in the observation group was significantly better than that of the control group(χ^(2)=8.904,P=0.031).The patients in the observation group presented significantly shorter pain relief time(t=3.402,P=0.001),and significantly lower incidence of postherpetic neuralgia(χ^(2)=4.835,P=0.028)than those of the control group.Conclusion For AHZ patients,intramuscular injection of Cobamamide,wIRA irradiation combined with PBN can improve treatment efficiency,significantly shorten pain relief time,and effectively reduce the incidence of postherpetic neuralgia.
