肛门袋冲洗结合低负压引流预防ICU大便失禁患者失禁性皮炎及压疮的效果观察及机制分析

摘　　要：目的研究肛门袋冲洗结合低负压引流对ICU大便失禁患者肛周皮肤损害程度、失禁性皮炎及压疮情况的影响。方法选取本院ICU2020年5月至2021年12月期间107例大便失禁患者作为研究对象,采用随机数字表法分组,对照组53例给予常规操作行失禁性皮炎护理,观察组54例在常规操作基础上应用肛门袋冲洗结合低负压引流,对比两组肛周皮肤损害程度、失禁性皮炎及压疮情况、医护人员工作量。结果干预后观察组患者失禁性皮炎及骶尾部压疮发生率均低于对照组;干预后观察组患者肛周皮肤损害程度低于对照组(P<0.05);观察组平均每天肛周护理次数、每日肛周护理时间、肛周皮肤愈合天数均低于对照组(P<0.05)。结论肛门袋冲洗结合低负压引流对ICU大便失禁患者效果确切,能够有效降低失禁性皮炎及骶尾部压疮发生率,预防肛周皮肤损害,减轻医护人员工作量,具有临床推广价值。Objective To study the effects of anal bag flushing combined with low negative pressure drainage on perianal skin damage,incontinence dermatitis and pressure ulcers in ICU patients with fecal incontinence.Methods A total of 107 patients with fecal incontinence in ICU from May 2020 to December 2021 in our hospital were selected as the research objects and divided into groups by random number table method.53 patients in the control group received routine operation for incontinence dermatitis nursing,while 54 patients in the observation group received anal bag irrigation combined with low negative pressure drainage on the basis of routine operation.The degree of perianal skin damage,incontinence dermatitis and pressure ulcers and the workload of medical staff were compared between the two groups.Results After intervention,the incidence of incontinence dermatitis and sacrococcal pressure ulcers in observation group was lower than that in control group.After intervention,the degree of perianal skin damage in observation group was lower than that in control group(P<0.05).The average number of perianal nursing times,perianal nursing time and perianal skin healing days in the observation group were lower than those in the control group(P<0.05).Conclusion Anal bag irrigation combined with low negative pressure drainage has a definite effect on ICU patients with fecal incontinence,which can effectively reduce the incidence of incontinence dermatitis and sacrococcal pressure ulcers,prevent perianal skin damage and reduce the workload of medical staff,and has clinical promotion value.
