摘 要:目的:探究及观察限制性液体复苏对失血性休克患者多系统器官功能及氧供氧耗的影响。方法:将2019年3月—2022年3月永安市立医院的80例失血性休克患者根据随机数字表法分为对照组和观察组,每组40例。对照组进行常规液体复苏治疗,观察组则进行限制性液体复苏治疗。比较两组治疗前后的肝功能指标[谷草转氨酶(AST)、谷丙转氨酶(ALT)]、肾功能指标[尿素氮(BUN)、肌酐(Cr)]、心肌损伤指标[肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)]及氧供氧耗指标[氧供(DO_(2))、氧供指数(DO_(2)I)、氧耗(VO_(2))、氧耗指数(VO_(2)I)]。结果:治疗前两组肝功能指标、肾功能指标、心肌损伤指标及氧供氧耗指标比较,差异均无统计学意义(P>0.05),治疗1、2、6 h两组的肝功能指标、肾功能指标及心肌损伤指标均升高,但观察组均低于对照组,氧供氧耗指标均升高,且观察组均高于对照组,差异均有统计学意义(P<0.05)。结论:限制性液体复苏可有效控制失血性休克患者的多系统器官功能损害,且对氧供氧耗的影响也更为积极有效,因此在失血性休克患者中的应用价值相对更高。Objective:To investigate and observe the influence of limited fluid resuscitation on multisystem organ function and oxygen supply and consumption of patients with hemorrhagic shock.Method:A total of 80 patients with hemorrhagic shock in Yong’an Municipal Hospital from March 2019 to March 2022 were divided into the control group and the observation group with 40 cases in each group by the method of random number table.The control group was treated with routine fluid resuscitation,the observation group was treated with limited fluid resuscitation.Then the liver function indexes[aspartate transaminase(AST)and alanine transaminase(ALT)],renal function indexes[blood urea nitrogen(BUN)and creatinine(Cr)],myocardial injury indexes[creatine kinase(CK)and creatine kinase isoenzymes(CK-MB)],oxygen supply and consumption indexes[oxygen supply(DO_(2)),oxygen supply index(DO_(2)I),oxygen consumption(VO_(2)),oxygen consumption index(VO_(2)I)]of two groups before and after treatment were compared.Result:The liver function indexes,renal function indexes and myocardial injury indexes,oxygen supply and consumption indexes of two groups before treatment were compared,the differences were not statistically significant(P>0.05),the liver function indexes,renal function indexes and myocardial injury indexes of two groups at 1st,2nd and 6th hour after treatment increased,while those of observation group were all lower than those of control group,the oxygen supply and consumption indexes increased,and those of observation group were significantly higher than those of control group,the differences were statistically significant(P<0.05).Conclusion:The limited fluid resuscitation can effectively control the multisystem organ dysfunction of patients with hemorrhagic shock,and its influence for oxygen supply and consumption is more active,so its application value in the patients with hemorrhagic shock is relatively higher.
