耳穴贴压联合穴位贴敷治疗急性胰腺炎临床研究

摘　　要：目的:观察耳穴贴压联合穴位贴敷治疗急性胰腺炎的临床疗效。方法:选取92例急性胰腺炎患者,按随机信封法分为对照组和研究组各46例。对照组给予常规西药治疗,研究组在对照组基础上联合穴位敷贴、耳穴贴压治疗。比较2组中医证候评分、改良CT严重程度指数(MCTSI)分级、实验室指标[白细胞计数(WBC)、C-反应蛋白(CRP)、血清淀粉酶(AMS)和脂肪酶(LPS)]以及临床疗效。结果:研究组总有效率为97.83%,高于对照组84.78%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组腹满硬痛、大便干结不通、日脯潮热、胸脘痞塞、呕吐、小便短赤等中医证候评分均较治疗前降低(P<0.05),且研究组上述各项评分均低于对照组(P<0.05)。治疗后,研究组MCTSI分级改善优于对照组,差异有统计学意义(P<0.05)。治疗后,2组WBC、CRP、AMS、LPS水平均较治疗前降低(P<0.05),且研究组上述各项指标水平均低于对照组(P<0.05)。结论:耳穴贴压联合穴位贴敷治疗急性胰腺炎可抑制机体炎症反应,改善患者中医证候及MCTSI病情分级,临床疗效显著。Objective:To observe the clinical effect of auricular point seed-pressing combined with point application for acute pancreatitis.Methods:A total of 92 patients with acute pancreatitis were selected and randomly divided into the control group and the study group according to the random envelope method,with 46 cases in each group.The control group was treated with conventional western medicine,and the study group was treated with point application and auricular point seed-pressing based on the treatment of the control group.The traditional Chinese medicine(TCM)syndrome scores,Modified CT Severity Index(MCTSI)grades,laboratory indicators[white blood cell count(WBC),C-reactive protein(CRP),serum amylase(AMS)and lipase(LPS)],and clinical effects were compared between the two groups.Results:The total effective rate was 97.83%in the study group,higher than that of 84.78%in the control group,the difference being significant(P<0.05).After treatment,the TCM syndrome scores such as abdominal fulness and hard pain,dry stool,hot flashes,chest congestion,vomiting,and scanty dark urine in the two groups were decreased when compared with those before treatment(P<0.05),and the above scores in the study group were lower than those in the control group(P<0.05).After treatment,the MCTSI grade Was superior to that in the control group,the difference being significant(P<0.05).After treatment,the levels of WBC,CRP,AMS,and LPS in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of the above indicators in the study group were lower than those in the control group(P<0.05).Conclusion:Auricular point seed-pressing combined with point application in the treatment of acute pancreatitis can inhibit the inflammatory responses of the body,improve the TCM syndromes and MCTSl grades,and have significant clinical effect.
