摘　　要：目的 探究抗人乳头瘤病毒(HPV)生物蛋白敷料联合普罗雌烯乳膏治疗绝经后女性宫颈HPV感染的治疗效果及安全性。方法 回顾性分析2018年3月—2022年8月于火箭军特色医学中心就诊的228例绝经后高危型人乳头瘤病毒(HR-HPV)感染女性,其中抗HPV生物蛋白敷料联合普罗雌烯乳膏治疗的患者为研究组共124例,保妇康栓治疗的患者为对照组共104例。采集患者一般资料,两组患者均治疗3个月,分别于治疗前及治疗3个月后检测患者液基细胞学技术(TCT)、HPV及阴道微生态指标。对比研究组及对照组治疗前后HPV转阴情况、阴道微生态指标及不良反应发生率。结果 两组患者年龄、绝经时间及HR-HPV感染亚型数量比较无统计学差异,研究组患者治疗后阴道微生态异常率、pH值及Nugent评分下降(P<0.05),研究组患者治愈率及总有效率分别为34.7%、65.3%,高于对照组(P<0.05),两组间不良反应发生率比较无统计学差异。结论 抗HPV生物蛋白敷料联合普罗雌烯乳膏可以改善阴道微生态从而促进绝经后女性HR-HPV转阴,其疗效确切、应用安全。OBJECTIVE To explore the effectiveness and safety of anti-human papillomavirus(HPV)bioprotein dressings combined with promestriene cream in treatment of postmenopausal women with cervical HPV infection.METHODS Totally 228 postmenopausal women with high-risk HPV(HR-HPV)infection who were treated in the PLA Rocket Force Characteristic Medical Center from Mar 2018 to Aug 2022 were retrospectively analyzed.124 patients who were treated with anti-HPV bioprotein dressings combined with promestriene cream were assigned as the study group,and 104 patientsr who were treated with Baofukangshuan were assigned as the control group.The baseline data were collected from the patients,both groups were treated for 3 months,Thinprep Cytologic Test(TCT),HPV and vaginal microecosystem indexes were detected before the treatment and after the treatment for 3 months.The result of HPV test,vaginal microecosystem indexes and incidence of adverse reactions were ob-served and compared between the study group and the control group before and after the treatment.RESULTS There were no significant differences in the age,menopausal time and number of subtypes of HR-HPV infection between the two groups.The abnormal rate of vaginal microecosystem,pH value and Nugent score of the study group were reduced after the treatment(P<0.05).The cure rate of the study group was 34.7%,higher than that of the control group;the total effective rate of the study group was 65.3%,higher than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.CONCLUSION The anti-HPV bioprotein dressings combined with promestriene cream may improve the vaginal microecosystem and facilitate the negative turn of HR-HPV of the postmenopausal women,it is highly effective and safe.
