摘　　要：副球孢子菌病是一种主要由巴西副球孢子菌感染引起的急性至慢性全身性真菌病,是最重要的系统性真菌病之一,高度流行于中南美洲,是巴西非免疫抑制个体死亡的主要原因,最近已被世界卫生组织(WHO)纳入被忽视疾病清单,其主要致病菌巴西副球孢子菌也位于WHO新发的首份“重点真菌病原体清单”中。该病通常被认为是农业工人、建筑工人的职业病,需要长期而复杂的治疗,具有严重的外皮临床表现,严重影响患者的生活和工作质量。传统认为其流行区域局限于中南美洲的部分地区,但随着全球人口流动的增加以及气候的变化,非疫区的输入性病例报道越来越多。最近也有不少研究对其传统流行病学观念提出了挑战。我国不是副球孢子菌病的流行区域,但已有副球孢子菌病输入性病例报道,因此有必要对其现状有所了解,尤其是从事真菌病、感染性疾病等相关工作者需密切关注。本综述在总结有关副球孢子菌病研究重要进展的基础上,重点介绍其最新的流行病学情况,并讨论来自分子研究以及与临床、诊断和治疗方面相关的新见解。Paracoccidioidomycosis is an acute to chronic systemic fungal disease mainly caused by Paracoccidioides brasiliensis infection.It is one of the most important systemic fungal diseases,highly prevalent in Central and South America,and a leading cause of death of non-immunosuppressive individuals in Brazil.Recently.it has been included in the List of Neglected Diseases by the World Health Organization(WHO),and Paracoccidioides bra-siliensis,as the major pathogenic bacterium,is also in the first"List of Key Fungal Pathogens"newly issued by the WHO.This disease is usually considered an occupational disease for agricultural and construction workers,re-quiring long-term and complex treatment,showing severe external skin clinical manifestations,and seriously af-fecting the quality of life and work of patients.Traditionally,it is believed that its epidemic area is limited to some parts of Central and South America,but an increasing number of reports of imported cases from non-epidemic are-as came with the increase of global population mobility and climate change.Recently,many studies have also posed challenges to its traditional concept of epidemiology.Although China is not an epidemic area of Paracoccid-ioidomycosis,there have been reports of imported cases of Paracoccidioidomycosis.So it is necessary to understand its current situation,especially for those who are engaged in research on the infectious fungal diseases.On the ba-sis of summarizing the important progress of research on Paracoccidioidomycosis,this review focuses on introdu-cing the latest epidemiological situation and discussing new insights from molecular research as well as clinical,di-agnostic,and therapeutic aspects.
