摘 要:目的 探讨富血小板血浆(PRP)治疗膝骨关节炎的临床疗效及对患者血清胃饥饿素、高迁移率族蛋白1(HMGB1)水平的影响。方法 选取2021年1月至2023年1月自贡市第一人民医院收治的膝骨关节炎患者106例为研究对象,根据治疗方法分为PRP治疗组和透明质酸治疗组,各53例。比较两组治疗效果及血清胃饥饿素、HMGB1水平。结果 PRP组治疗后1、3个月膝关节损伤及骨关节炎调查问卷(KOOS)评分、Lysholm评分、骨密度高于透明质酸组,差异有统计学意义(t=3.235、10.437、5.494、3.863、4.183、10.268,P<0.05);PRP组治疗后1、3个月胃饥饿素水平高于透明质酸组,差异有统计学意义(t=2.065、3.611,P<0.05);HMGB1水平低于透明质酸组,差异有统计学意义(t=2.193、6.146,P<0.05);PRP组治疗后1、3个月视觉模拟评分(VAS)评分低于透明质酸组,生活质量评价量表(SF-36)评分高于透明质酸组,差异有统计学意义(t=3.119、6.661、2.713、3.550,P<0.05);PRP组及透明质酸组总不良反应发生率分别为9.43%、13.21%,两组不良反应发生率比较差异无统计学意义(χ^(2)=0.376,P>0.05)。结论 PRP可以提高血清胃饥饿素水平,降低HMGB1水平,改善膝骨关节炎患者关节功能,缓解疼痛提高生活质量。Objective To investigate the effect of platelet-rich plasma(PRP)on the level of se-rum ghrelin and high mobility group protein 1(HMGB1)in the treatment of knee osteoarthritis.Methods A total of 106 patients with knee osteoarthritis treated at Zigong First People's Hospital from January 2021 to Jan-uary 2023 were selected as the study subjects.They were divided into two groupos based on treatment meth-ods:the PRP treatment group and the hyaluronic acid treatment group,each consisting of 53 cases.The study compared the therapeutic effects and serum levels of ghrelin and HMGB1 between the two groups.Results The Knee Injury and Osteoarthritis Questionnaire(KOOS)scores,Lysholm scores and bone mineral density in the PRP group were higher than those in the hyaluronic acid group at 1 and 3 months after treatment,with statistical significance(t=3.235,10.437,5.494,3.863,4.183,10.268,P<0.05).The level of ghrelin in the PRP group was higher than that in the hyaluronic acid group at 1 and 3 months after treatment,and the differ-ence was statistically significant(t=2.065,3.611,P<0.05).HMGB1 level was lower than that in the hyaluron-ic acid group,and the difference was statistically significant(t=2.193,6.146,P<0.05).The Visual Analogue Scale(VAS)score in the PRP group was lower than that in the hyaluronic acid group at 1 and 3 months after treatment,and the SF-36 score in the PRP group was higher than that in the hyaluronic acid group,with statis-tical significance(t=3.119,6.661,2.713,3.550,P<0.05).The incidence of adverse reactions in the PRP group and hyaluronic acid group was 9.43%and 13.21%,respectively,and there was no statistical signifi-cance in the incidence of adverse reactions between the two groups(χ^(2)=0.376,P>0.05).Conclusion PRP can increase serum ghrelin levels,decrease HMGB1 levels,improve joint function,relieve pain and enhance the quality of life in patients with knee osteoarthritis.
