摘  要:目的 分析血清维生素D、白介素-6(IL-6)、白介素-8(IL-8)水平与早产儿支气管肺发育不良(BPD)的相关性。方法 选择2020年1月至2023年1月于唐山市妇幼保健院三级新生儿重症监护病房出生的早产儿(胎龄≤35周,住院时长≥28 d)216例,依据BPD诊断标准分为BPD组(n=120)、非BPD组(n=96)。比较两组第1、7、14、21、28 d血清1,25-二羟维生素D3[1,25-(OH)2D3]、IL-6、IL-8水平变化。比较BPD组出院时不同病情程度血清1,25-(OH)2D3、IL-6、IL-8水平变化。分析血清1,25-(OH)2D3、IL-6、IL-8水平与FiO2水平变化的相关性。结果 随着时间推移,两组血清1,25-(OH)2D3水平均在第21 d降到最低值,且BPD组在各时间点血清1,25-(OH)2D3水平均低于非BPD组,差异具有统计学意义(P<0.05)。随着时间推移,两组血清IL-6、IL-8水平分别在第14、21 d达到顶峰,且BPD组各时间血清IL-6、IL-8水平均高于非BPD组,差异具有统计学意义(P<0.05)。1,25-(OH)2D3水平:重度<中度<轻度,IL-6、IL-8水平:重度>中度>轻度,差异具有统计学意义(P<0.05)。以血清1,25-(OH)2D3、IL-6、IL-8为因变量,经Pearson’s Correlation皮尔逊相关性结果得知,血清1,25-(OH)2D3与FiO2水平呈负相关,血清IL-6、IL-8水平与FiO2水平呈正相关(P<0.05)。结论 血清维生素D、IL-6及IL-8水平与早产儿BPD发生发展具有一定联系,动态监测上述水平有利于评估早产儿BPD病情发展,并为临床实践中早期发现和干预BPD提供有价值的参考信息。Objective To analyze the correlation between serum levels of vitamin D,interleukin-6(IL-6),interleukin-8(IL-8)and bronchopulmonary dysplasia(BPD)in preterm infants in order to find more effective treatment.Methods A total of 216 premature infants(gestational age≤35 weeks,length of stay≥28 days)born in the tertiary neonatal intensive care unit at Tangshan Maternal and Child Health Hospital from January 2020 to January 2023 were selected and divided into a BPD group(n=120)and a non-BPD group(n=96)according to the diagnostic criteria for BPD.The study aimed to compare the changes in serum 1,25-dihy-droxyvitamin D3,1,25-(OH)2D3,IL-6,and IL-8 levels on the 1st,7 th,14 th,21st,and 28 th days be-tween the two groups.Additionally,the research examined the changes in serum 1,25-(OH)2D3,IL-6,and IL-8 levels in the BPD group at the time of discharge from the hospital with different degrees of the disease.Further-more,the study analyzed the correlation between serum 1,25-(OH)2D3,IL-6,and IL-8 levels and changes in FiO2 levels.Results Over time,serum 1,25-(OH)2D3 levels in both groups decreased to their lowest value at the 21st day,with levels in the BPD group consistently lower than those in the non-BPD group at all-time points.This difference was statistically significant(P<0.05).Similarly,serum IL-6 and IL-8 levels peaked at the 14th and 21st day in both groups,with levels in the BPD group consistently higher than those in the non-BPD group at all times.This difference was statistically significant(P<0.05).The order of severity for 1,25-(OH)2D3 level was:severe<moderate<mild,IL-6 and IL-8 levels:severe>moderate>mild,the difference was statistically significant(P<0.05).According to Pearson's Correlation results,serum 1,25-(OH)2D3 was negatively correlated with FiO2 levels,and serum IL-6 and IL-8 levels were positively correlated with FiO2 levels(P<0.05).Conclusion Serum levels of vitamin D,IL-6 and IL-8 are associated with the development of BPD in premature infants.Regular monitoring of these levels helps evaluate prog
