摘 要:目的 分析急性心肌梗死(AMI)患者血清微小RNA-21(miRNA-21)变化及其与细胞凋亡因子[半胱氨酸天冬酶-3(Caspase-3)、可溶性肿瘤坏死因子相关性凋亡诱导配体(sTRAIL)、可溶性凋亡相关因子(sFas)]的相关性。方法 选取北京市大兴区人民医院2020年4月至2022年4月112例AMI患者作为研究组,根据冠脉病变程度分为单支组(n=30)、双支组(n=48)及三支组(n=34),选取同期健康体检者70名作为对照组。采用实时荧光定量聚合链式反应技术检测血清miRNA-21水平,酶联免疫吸附法检测血清Caspase-3、sTRAIL及sFas水平。比较所有受试者血清miRNA-21、Caspase-3、sTRAIL及sFas水平,采用Pearson系数分析miRNA-21与Caspase-3、sTRAIL及sFas的相关性,采用ROC曲线分析血清miRNA-21、Caspase-3、sTRAIL及sFas对AMI的诊断价值,采用Spearman系数分析miRNA-21、Caspase-3、s TRAIL及s Fas与冠脉病变程度的相关性。结果 研究组血清miRNA-21、Caspase-3、sTRAIL及sFas水平均高于对照组,差异均有统计学意义(P<0.05);Pearson相关性分析显示,AMI患者血清miRNA-21均与Caspase-3、sTRAIL、sFas呈正相关(P<0.05);由ROC曲线可知,血清miRNA-21、Caspase-3、sTRAIL及s Fas联合诊断AMI的AUC为0.940,高于四者单独诊断的0.788、0.749、0.775、0.756(P<0.05);三组血清miRNA-21、Caspase-3、sTRAIL及sFas水平相比:三支组>双支组>单支组,差异均有统计学意义(P<0.05);Spearman相关性分析显示,miRNA-21、Caspase-3、sTRAIL及sFas均与冠脉病变程度呈正相关(P<0.05)。结论 miRNA-21与细胞凋亡因子在AMI患者血清中呈高表达,且均与冠脉病变程度相关,有望成为诊断AMI的参考指标。Objective To analyze the changes of serum microRNA-21(miRNA-21)and its corre-lation with apoptosis factors[cysteine proteinase-3(caspase-3),soluble tumor necrosis factor-related apoptosis-inducing ligand(sTRAIL),soluble apoptosis-related factor(sFas)]in patients with acute myocardial infarc-tion(AMI).Methods A total of 112 patients with AMI at Daxing District Peoples Hospital,Beijing,were enrolled as the study group between April 2020 and April 2022.Based on the severity of their coronary le-sions,they were divided into three groups:single-vessel group(n=30),double-vessel group(n=48)and three-vessel group(n=34).A total of 70 healthy individuals during the same period were enrolled as the control group.The levels of serum miRNA-21 were measured using real-time fluorescence quantitative polymerase chain reaction.The levels of serum caspase-3,sTRAIL and sFas were measured using enzyme-linked immuno-sorbent assay.The levels of serum miRNA-21,caspase-3,sTRAIL and sFas were compared among all partici-pants.The correlation between miRNA-21 and caspase-3,sTRAIL,and sFas was analyzed using Pearson coef-ficient.The diagnostic value of serum miRNA-21,caspase-3,sTRAIL and sFas for AMI was determined using ROC curves,and their correlation with the severity of coronary lesions was analyzed using the Spearman coeffi-cient.Results The levels of serum miRNA-21,caspase-3,sTRAIL and sFas in the study group were higher than those in the control group(P<0.05).Pearson correlation analysis showed that serum miRNA-21 was posi-tively correlated with caspase-3,sTRAIL and sFas(P<0.05).The ROC curve analysis showed that the AUC of miRNA-21 combined with Caspase-3,sTRAIL and sFas in the diagnosis of AMI was 0.940,which was greater than that of asingle indicator(0.788,0.749,0.775,0.756,P<0.05).The levels of serum miRNA-21,caspase-3,sTRAIL and sFas were the highest in the three-vessel group,followed by the double-vessel group and the single-vessel group(P<0.05).Spearman correlation analysis showed that miRNA-21,caspase-3,sTRAIL and sFas were p
