腹壁浅静脉血流反向对PTS患者髂静脉慢性梗阻的诊断价值

摘　　要：目的本研究旨在探讨腹壁浅静脉(SEV)血流反向对深静脉血栓形成后综合征(PTS)患者髂静脉慢性梗阻的诊断价值。方法回顾性分析23例超声发现SEV血流反向的PTS患者的下肢静脉声像图,并与其他影像学进行对比,同时记录患者人口统计学资料、深静脉血栓形成(DVT)史、CEAP C评分、Villalta评分、梗阻部位及SEV走行方向。结果超声发现SEV血流反向的PTS患者的髂静脉均有较明显狭窄或闭塞,并经其他影像学证实;通过追踪SEV的走行发现有多种侧支通路发挥下肢静脉代偿引流作用。结论超声发现SEV血流反向是诊断PTS患者髂静脉慢性梗阻的一种简便可靠方法;SEV及其侧支是髂静脉慢性梗阻患者下肢静脉回流的重要代偿通路。Objective To explore the application value of superficial epigastric vein(SEV)flow reversal in the diagnosis of chronic iliacaval obstruction in post thronbotic syndrome(PTS)patients.Methods The lower extremity veins ultrasound images in 23 PTS patients who were found flow reversal within SEV were analyzed retrospectively,and compared with other images.At the same time,the demographic data,history of deep vein thrombosis(DVT),CEAP C score,Villalta score,obstruction site and SEV direction were recorded.Results Ultrasound found that the iliacaval of PTS patients with SEV flow reversal were obviously narrowed or occluded,which was confirmed by other imaging.At the same time,by tracing the SEV,it is found that there are several kinds of collateral pathways to play the role of compensatory drainage of lower extremity veins.Conclusions It is a simple and reliable method to diagnose of chronic iliacaval obstruction in PTS patients according to the ultrasound finding of SEV flow reversal.SEV and its collateral branches are important compensatory pathways for venous return of lower limbs in patients with chronic iliacaval obstruction.
