湿包疗法在成人中重度特应性皮炎及其他皮炎湿疹类皮肤病中的疗效观察

摘　　要：目的观察湿包疗法(WWT)在特应性皮炎及其他皮炎湿疹类皮肤病中的疗效及安全性。方法回顾分析2015年1月—2023年5月在首都医科大学附属北京友谊医院皮肤科住院期间应用WWT治疗的特应性皮炎(AD)及其他皮炎湿疹类皮肤病患者,总结WWT的疗效及不良反应。结果共收集44例患者,病种包括AD、红皮病、嗜酸性粒细胞增多性皮炎、湿疹、AD结节痒疹型、光敏性皮炎以及药疹。治疗后患者湿疹面积及严重程度指数(EASI)、研究者总体评估(IGA)、体表受累面积(BSA)、瘙痒-数字评价量表(NRS)均得到显著改善,外周血嗜酸性粒细胞水平较前下降。AD结节痒疹型组WWT应用时间显著高于AD、红皮病和湿疹组。应用0.05%卤米松乳膏进行WWT组的EASI评分下降率显著高于0.1%糠酸莫米松乳膏组和青鹏软膏组。应用0.05%卤米松乳膏进行WWT组患者治疗后IGA评分显著低于青鹏软膏组。结论WWT治疗成人中重度AD及皮炎湿疹类疾病的疗效肯定,安全性好。Objective To observe the efficacy and safety of wet-wrap therapy in adults with moderate to severe atopic dermatitis and other dermatitis and eczema.Methods We retrospectively reviewed clinical records and laboratory results of adult patients with moderate to severe atopic dermatitis and other dermatitis and eczema who were received wet-wrap therapy(WWT)and hospitalized in the Department of Dermatology,Beijing Friendship Hospital affiliated to Capital Medical University from January 2015 to May 2023.The efficacy and adverse reactions were observed before and after treatment.Results A total of 44 patients were collected,and the diagnosis included AD,erythroderma,hypereosinophilic dermatitis,eczema,prurigo nodularis,photosensitive dermatitis and drug eruption.After treatment,the patient′s eczema area and severity index(EASI)score,investigator′s global assessment(IGA)score,body surface area(BSA)and itch numeric rating scale(NRS)score were significantly improved,and peripheral blood eosinophil cells′level decreased compared with before.The application time of WWT in prurigo nodularis group was significantly longer than that in AD,erythroderma and eczema group,respectively.After treatment,the reduction rate of EASI score in WWT treated with halometasone cream 0.05%group was significantly higher than that in Mometasone Furoate cream 0.1%group and Qingpeng ointment group,respectively.The IGA score in WWT treated with halometasone cream 0.05%group was significantly lower than that in Qingpeng ointment group after treatment.Conclusion WWT is an effective and safe treatment for moderate to severe AD,dermatitis and eczema in adults.
