中药熏洗坐浴法结合挂线手术治疗肛瘘的临床疗效研究

摘　　要：分析肛瘘患者联合应用中药熏洗坐浴法与挂线手术治疗的临床效果。以2020年10月—2022年10月收治的112例肛瘘患者为研究对象,经随机数表法将其分为对照组(n=56,实施挂线手术治疗)和观察组(n=56,实施中药熏洗坐浴法结合挂线手术治疗),采用统计学分析两组治疗效果、并发症发生情况、肛门动力学变化、生活质量及负性情绪情况。结果显示,观察组治疗总有效率高于对照组(P<0.05);观察组并发症发生率经统计学计算显示低于对照组(P<0.05)。治疗前,两组患者肛门动力学指标比较,无显著差异(P>0.05);治疗后,两组患者肛门动力学指标中直肠静息压和肛管静息压均降低,且观察组低于对照组(P<0.05);治疗后,SF-36中社会及生理功能等8个维度评分比较,观察组均高于对照组(P<0.05);观察组SAS、SDS评分均低于对照组(P<0.05)。研究发现,以挂线手术治疗为基础实施中药熏洗坐浴法治疗肛瘘患者临床疗效显著,有利于改善患者肛门动力学,且可有效预防各类并发症,提高患者生活质量,减少其负性情绪,建议推广应用。Analyze the clinical effects of combining traditional Chinese medicine fumigation and sitz bath with thread hanging surgery in the treatment of analfistula patients.112 patients with analfistula treated from October 2020 to October 2022 were selected as the study subjects.They were randomly divided into a control group(n=56,treated with thread hanging surgery)and an observation group(n=56,treated with traditional Chinese medicine fumigation and sitz bath combined with thread hanging surgery)using a random number table method.The treatment effects,incidence of complications,changes in anal dynamics,quality of life,and negative emotions of the two groups were statistically analyzed.The total effective rate of the observation group was higher than that of the control group(P<0.05).The incidence of complications in the observation group was statistically lower than that in the control group(P<0.05).Before treatment,there was no significant difference in anal motility indicators between the two groups of patients(P>0.05).After treatment,the rectal and anal resting pressure in the anal motility indicators of the two groups of patients decreased,and the observation group was lower than the control group(P<0.05).After treatment,the social and physiological function scores of SF-36 in 8 dimensions were compared,and the observation group was higher than the control group(P<0.05).The SAS and SDS scores of the observation group were lower than those of the control group(P<0.05).Research has found that the implementation of traditional Chinese medicine fumigation and sitz bath based on thread hanging surgery has significant clinical efficacy in treating patients with analfistula,which is beneficial for improving anal dynamics,effectively preventing various complications,improving patient quality of life,and reducing negative emotions.It is recommended to promote its application.
