摘　　要：目的:探究血栓弹力图检测应用在肝癌患者输血中的临床价值。方法:纳入2022年1月~2023年7月本院收治的肝癌术后凝血功能异常需输血治疗的患者为观察对象。将80例病例进行简单随机分组处理,对照组行常规凝血试验,观察组行血栓弹力图检测,对输注前后的凝血功能、输血量、出血量以及出血时间方面进行组间差异性比较,分析血栓弹力图检测的可行性价值。结果:观察组和对照组输注后凝血四项和血小板计数均发生明显改善,相比对照组,观察组活化部分凝血活酶时间、血浆凝血酶时间、凝血酶时间水平均低,血浆纤维蛋白原、血小板计数水平均高(P<0.05);观察组出血时间短、出血量少于对照组(P<0.05);观察组新鲜冰冻血浆、红细胞、冷沉淀凝血因子输注量比对照组更少(P<0.05)。结论:血栓弹力图检测在肝癌术后患者输血指导中的应用意义十分突出。Objective:To explore the clinical value of thromboelastography in blood transfusion of patients with liver cancer.Methods:This study was carried out in our hospital,and the subjects were all patients with abnormal coagulation function after liver cancer surgery who needed blood transfusion treatment.80 cases were randomly divided into two groups,the control group was given routine coagulation test,and the observation group was given thromboelastography.The differences between the two groups were compared in coagulation function,blood transfusion volume,bleeding volume and bleeding time before and after infusion,and the feasibility value of thromboelastography detection was analyzed.Results:After infusion,the four coagulation items and platelet count in the observation group and the control group were significantly improved.Compared with the control group,the levels of APTT,PT and TT in the observation group were lower,while the levels of FIb and PLT were higher(P<0.05).The bleeding time in the observation group was shorter and the amount of bleeding was less than that in the control group(P<0.05).The infusion of FFP,CRC and CRYO in the observation group was less than that in the control group(P<0.05).Conclusion:The application of thromboelastography in blood transfusion guidance for patients with liver cancer after operation is very significant.
