微创经皮钢板内固定双钢板技术治疗胫骨平台骨折临床疗效观察

摘  要:目的:针对SchatzkerⅤ、Ⅵ型胫骨平台骨折,观察采用微创经皮钢板内固定(MIPPO)双钢板技术治疗的临床疗效,以评估其使用价值。方法:将2020年2月~2023年5月在贵州省织金县人民医院确诊的60例SchatzkerⅤ、Ⅵ型胫骨平台骨折患者,其中28例采用锁定钢板治疗,设为对照组,其中32例采用MIPPO双钢板技术治疗,设为观察组。两组患者均完成6个月以上的术后随访,设定四项观察指标,分别为手术情况比较、骨折愈合情况、膝关节活动度、术后并发症发生情况。结果:观察组在手术时间(88.52±8.37)min、术中出血量(282.53±51.28)mL较对照组手术时间(90.15±9.62)min、术中出血量(279.85±49.85)mL无明显差异,无统计学意义(P>0.05)。观察组在骨痂形成时间(6.55±0.71)周、骨折愈合时间(11.46±0.25)周较对照组骨痂形成时间(7.87±0.98)周、骨折愈合时间(13.42±0.37)周短,差异均有统计学意义(P<0.05)。观察组在膝关节伸直度(6.32˚±0.41˚)、膝关节屈曲度(93.25˚±14.62˚)优于对照组膝关节伸直度(8.61˚±0.63˚)、膝关节屈曲度(80.13˚±10.15˚),差异有统计学意义(P<0.05)。观察组在扶拐负重时间(7.23±0.45)周、完全负重时间(13.89±1.22)周较对照组扶拐负重时间(7.32±0.59)周、完全负重时间(14.01±1.77)周无明显差异,无统计学意义(P>0.05)。观察组并发症发生率9.37%(3/32)低于对照组的32.14%(9/28),差异有统计学意义(P<0.05)。结论:针对SchatzkerⅤ、Ⅵ型胫骨平台骨折,MIPPO双钢板技术治疗胫骨平台骨折效果显著。该疗法不仅能够加快骨折愈合,改善膝关节功能,还能明显降低并发症发生率。Objective:To observe the clinical efficacy of minimally invasive percutaneous plate internal fixation(MIPPO)dual plate technique in the treatment of SchatzkerⅤandⅥtype tibial plateau fractures,in order to evaluate its value.Methods:60 patients with Schatzker type V andⅥtibial plateau fractures diagnosed in Zhijin County People's Hospital,Guizhou Province from February 2020 to May 2023 were enrolled.Among them,28 patients were treated with locking plates and assigned to the control group.Among them,32 patients were treated with MIPPO double plate technique and assigned to the observation group.Both groups of patients completed postoperative follow-up for more than 6 months,with four observation indicators set:comparison of surgical conditions,fracture healing,knee joint range of motion,and incidence of postoperative complications.Results:There was no significant difference in surgical time of(88.52±8.37)minutes and intraoperative blood loss of(282.53±51.28)mL between the observation group and the control group at(90.15±9.62)minutes and(279.85±49.85)mL,with no statistical significance(P>0.05).The observation group had a shorter callus formation time of(6.55±0.71)weeks and a fracture healing time of(11.46±0.25)weeks compared to the control group,with a callus formation time of(7.87±0.98)weeks and a fracture healing time of(13.42±0.37)weeks.The differences were statistically significant(P<0.05).The observation group had better knee joint extension(6.32˚±0.41˚)and knee joint flexion(93.25˚±14.62˚)than the control group in knee joint extension(8.61˚±0.63˚)and knee joint flexion(80.13˚±10.15˚),with statistical significance(P<0.05).There was no significant difference between the observation group and the control group in terms of weight-bearing time of(7.23±0.45)weeks and complete weight-bearing time of(13.89±1.22)weeks,with no statistical significance(P>0.05).The incidence of complications in the observation group was 9.37%(3/32)lower than that in the control group,which was32.14%(9/28),and t
