针刺联合柴术理胃饮治疗老年脾虚气滞型功能性消化不良临床观察

摘　　要：目的探讨针刺联合柴术理胃饮治疗老年脾虚气滞型功能性消化不良(FD)的临床疗效及安全性。方法选取2022年1月—2023年1月就诊于黑龙江省中医药科学院南岗院区老年病科确诊为脾虚气滞型FD 90例老年患者。采用乱数表法将90例患者随机分为治疗组和对照组,45例/组。治疗组采用针刺联合柴术理胃饮治疗,对照组采用吗丁啉治疗。治疗4周后,观察两组主症与次症积分、中医证候疗效、生存质量量表(FDDQL)、复发率,并对两组进行安全性评价。结果治疗4周后,两组主症与次症积分均显著下降(P<0.05);治疗组主症与次症积分明显低于对照组(P<0.05)。治疗组中医证候疗效有效率为91.11%(41/45),对照组为75.56%(34/45),治疗组临床疗效明显优于对照组(P<0.05)。两组治疗后FDDQL评分均较治疗前升高(P<0.05);治疗组评分明显高于对照组(P<0.05)。治疗结束3个月后,治疗组复发率为7.32%(3/41),对照组为32.35%(11/34),治疗组复发率明显低于对照组(P<0.05)。对照组轻度失眠2例,两组比较差异无统计学意义(P>0.05)。结论针刺联合柴术理胃饮治疗老年脾虚气滞型FD临床疗效显著,安全性极好。Objective To investigate the clinical efficacy and safety of acupuncture combined with Chaizhu Liwei Yin(柴术理胃饮)in the treatment of senile functional dyspepsia(FD)with spleen deficiency and Qi stagnation type.Methods From January 2022 to January 2023,90 elderly patients with FD of spleen deficiency and Qi stagnation type were selected from the department of geriatrics,Nangang hospital,Heilongjiang academy of traditional Chinese medicine.90 patients were randomly divided into treatment group and control group by random number table method,45 cases/group.The treatment group was treated with acupuncture combined with Chaizhu Liwei Yin,and the control group was treated with domperidone.After 4 weeks of treatment,the scores of main symptoms and secondary symptoms,the efficacy of TCM symptoms,the quality of life scale(FDDQL)and the recurrence rate were observed,and the safety of the two groups was evaluated.Results After 4 weeks of treatment,the scores of main symptoms and secondary symptoms in the two groups were significantly decreased(P<0.05).The scores of main symptoms and secondary symptoms in the treatment group were significantly lower than those in the control group(P<0.05).The effective rate of TCM symptoms was 91.11%(41/45)in the treatment group and 75.56%(34/45)in the control group.The clinical efficacy of the treatment group was significantly better than that of the control group(P<0.05).The FDDQL scores of the two groups after treatment were higher than those before treatment(P<0.05).The score of the treatment group was significantly higher than that of the control group(P<0.05).After 3 months of treatment,the recurrence rate was 7.32%(3/41)in the treatment group and 32.35%(11/34)in the control group.The recurrence rate in the treatment group was significantly lower than that in the control group(P<0.05).There was no significant difference in mild insomnia between the two groups(P>0.05).Conclusion Acupuncture combined with Chaizhu Liwei Yin is effective and safe in the treatment of senile FD with
